多层螺旋CT直接和间接血管成像对髂静脉压迫综合征及继发血栓形成的诊断价值

摘　　要：目的直接法多层螺旋CT(MSCT)静脉成像和间接法MSCT动脉成像是诊断髂静脉压迫综合征和下肢静脉血栓常用的方法,进一步比较分析这两种检查方法的优缺点,为优化临床检查提供依据。方法回顾性分析2014至2017年经DSA证实的髂静脉压迫综合征(IVCS)患者87例,同时有完善的间接法MSCT动脉成像(MSCTA)或直接法MSCT静脉成像(MSCTV)影像学资料。按检查方式分成2组,直接法髂静脉MSCTV组和间接髂静脉MSCTA组,比较两种检查方法在诊断IVCS组和其继发下肢血栓的诊断价值。结果直接法髂静脉MSCTV组和间接髂静脉MSCTA组患者性别、年龄、身高、体重差异无统计学意义(P>0.05),两种检查方法均能很好的诊断髂静脉受压综合征。直接法髂静脉MSCTV相比间接髂静脉MSCTA在诊断髂静脉、股静脉、腘静脉继发血栓更为清晰、敏感,差异有统计学意义(P<0.05)。结论MSCT是IVCS重要的检查手段,MSCTV和MSCTA均能很好诊断诊断IVCS,但MSCTV对下肢静脉继发血栓的显示更具优势,是临床除DSA之外相对简单、无创、安全的检查方法。Objective The direct MSCT venography and indirect method MSCT arterial imaging are common methods which are used in diagnosis of iliac vein compression syndrome(IVCS) and lower extremity venous thrombosis. This study would further analyze the advantages and disadvantages of the two inspection methods in order to provide the basis for optimizing the clinical examination. Methods The clinical data about 87 patients with IVCS who were confirmed by DSA in our hospital from 2014 to 2017 were retrospective analyzed,and the patients had complete iconography data of indirect MSCT arterial imaging(MSCTA) or direct MSCT venography(MSCTV). The patients were divided into direct iliac vein MSCTV group and indirect iliac vein MSCTA group according to the test mode. The diagnostic value of the two methods in diagnosis of IVCS and secondary lower limb venous thrombus was observed and compared. Results There were no significant differences in gender,age,height and weight between the two groups(P > 0. 05). The two methods were good for the diagnosis of IVCS.The direct iliac vein MSCTV was more clear and sensitive in diagnosing the secondary thrombosis of iliac vein,femoral vein and popliteal vein than those by indirect iliac vein MSCTA(P < 0. 05). Conclusion MSCT is an important examine method for IVCS,and both MSCTV and MSCTA can diagnose IVCS well,however MSCTV has more advantages in diagnosis of lower limb vein secondary thrombosis,which is more simple,noninvasive,safe inspection methods besides DSA in clinical practice.
