风湿祛痛胶囊联合双氯芬酸钠治疗强直性脊柱炎临床研究

摘　　要：目的观察风湿祛痛胶囊联合双氯芬酸钠治疗强直性脊柱炎的疗效及对免疫球蛋白和炎性因子的影响。方法选取医院2014年8月至2017年1月收治的强直性脊柱炎患者86例,按随机数字表法分为对照组(41例)和观察组(45例)。两组患者在常规对症治疗基础上均服用双氯芬酸钠缓释片(每次0.1g,每日1次),观察组患者加服风湿祛痛胶囊(每次5粒,每日3次),均治疗8周。结果治疗后,两组患者腰骶脊背和关节疼痛评分、巴氏强直性脊柱炎活动指数评分和巴氏强直性脊柱炎功能指数评分,免疫球蛋白A(IgA)和免疫球蛋白G,以及肿瘤坏死因子-α、C反应蛋白、白细胞介素4、白细胞介素23水平均明显下降,且观察组明显低于对照组(P<0.05);两组患者治疗后的免疫球蛋白M水平无明显差异(P>0.05);观察组总有效率为93.33%,明显高于对照组的75.61%(P<0.05);观察组与对照组不良反应发生率相当(11.11%比17.07%,P>0.05)。结论风湿祛痛胶囊联合双氯芬酸钠治疗强直性脊柱炎,疗效显著,可缓解疼痛程度,调节免疫球蛋白水平,降低血清炎性因子水平,且不增加不良反应。Objective To observe the clinical efficacy of Fengshi Qutong Capsules combined with diclofenac sodium in the treatment of ankylosing spondylitis and its effect on immunoglobulin and inflammatory factors.Methods Totally 86 patients with ankylosing spondylitis admitted to our hospital from August 2014 to January 2017 were selected and divided into the control group(n=41)and the observation group(n=45)according to the random number table method.The patients in the two groups were given Diclofenac Sodium Sustained-Release Tablets(0.1 g each time,once a day)on the basis of routine symptomatic treatment,on this basis,the patients in the observation group were given Fengshi Qutong Capsules(5 Capsules each time,3 times a day).All patients were treated for 8 weeks.Results After treatment,the scores of lumbosacral back and joint pain,papanicolaou ankylosing spondylitis activity index and papanicolaou ankylosing spondylitis functional index,and the levels of immunoglobulin A(IgA),IgG,tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-4(IL-4)and IL-23 in the two groups were significantly decreased,and those in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the level of IgM between the two groups after treatment(P>0.05).The total effective rate of the observation group was 93.33%,which was significantly higher than 75.61%of the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(11.11%vs.17.07%,P>0.05).Conclusion Fengshi Qutong Capsules combined with diclofenac sodium are effective in the treatment of ankylosing spondylitis,which can relieve pain,regulate the level of immunoglobulin and reduce the level of serum inflammatory factors without increasing the adverse reactions.
