某院含阿托伐他汀钙片门诊处方点评与分析

摘　　要：目的为临床合理应用阿托伐他汀钙片提供参考。方法从医院临床药学系统选取2017年10月至12月门诊含阿托伐他汀钙片处方2262份,对患者性别、年龄、联合用药及处方不合理问题进行统计和分析。结果2262份处方中,41~60岁和61~80岁患者使用阿托伐他汀钙片比例较高,分别占26.61%和56.63%;主要分布在心血管内科、神经内科、内分泌科,分别占43.37%,30.64%,7.21%;4种药物联用和5种药物联用分别占25.82%和24.23%;有436份处方存在不合理用药情况,其中诊断不规范或无诊断187份(42.89%),与适应证不符103份(23.62%),重复用药88份(20.18%),联合用药不适宜40份(9.17%)。结论该院阿托伐他汀钙片的临床使用基本合理,但仍有不规范现象,还存在一些问题,医院应加强监督,从源头上杜绝不合理用药,保障临床安全用药。Objective To provide reference for rational use of Atorvastatin Calcium Tablets in the clinic.Methods Totally 2 262 outpatient prescriptions containing Atorvastatin Calcium Tablets from October to December 2017 were selected from the clinical pharmacy system in our hospital,the gender and age of the patients,combined use of drugs and unreasonable prescriptions were analyzed.Results Among the 2 262 prescriptions,the proportion of patients aged 41-60 and 61-80 treated with Atorvastatin Calcium Tablets was higher,accounting for 26.61%and 56.63%respectively,mainly distributed in cardiovascular medicine,neurology and endocrinology,accounting for 43.37%,30.64%and 7.21%.The combined use of the 4 drugs and the 5 drugs accounted for 25.82%and 24.23%.Irrational drug use was found in 436 prescriptions,among which 187 cases(42.89%)with non-standard diagnosis or non-diagnosis,103 cases(23.62%)were inconsistent with indications,88 cases(20.18%)with repeat prescription,and 40 cases(9.17%)with inappropriate combination of drugs.Conclusion The clinical use of Atorvastatin Calcium Tablets in our hospital is basically reasonable,but there are still some irregular phenomena and problems.The hospital should strengthen supervision,eliminate irrational drug use from the source to ensure the safety of clinical drug use.
