摘  要:目的:探讨锁定钢板内固定治疗复杂胫骨平台骨折的临床效果及安全性。方法:选取2015年1月-2017年1月笔者所在医院骨科门诊收治的复杂性胫骨平台骨折患者100例作为研究对象。严格运用随机原则将其分为两组,对照组50例行传统空心钉内固定,观察组50例采取锁定钢板内固定手术,记录并比较两组手术时间、术中失血量、骨折愈合时间及并发症发生情况,依据Rasmussen评分量表评价临床疗效。结果:观察组术中失血量、骨愈合时间和负重功能锻炼时间均优于对照组,差异均有统计学意义(P<0.05)。观察组优良率(94.0%)明显高于对照组(70.0%),差异有统计学意义(P<0.05)。结论:较传统的传统的空心钉内固定,锁定钢板内固定治疗复杂性胫骨平台骨折创伤小,更利于促进术后骨愈合,固定牢靠,符合生物学固定原则,值得临床推广应用。Objective:To investigate the clinical efficacy and safety of locking plate internal fixation in the treatment of complex tibial plateau fractures. Method:From January 2015 to January 2017,100 patients with complex tibia plateau fractures treated in orthopedic clinic of the author's hospital were selected as the subjects of the study.According to the principle of randomization,50 cases in the control group were treated with traditional hollow nail internal fixation, and 50 cases in the observation group were treated with locking plate internal fixation.The operation time,intraoperative blood loss,fracture healing time and complications were recorded and compared between the two groups.The clinical efficacy was evaluated according to Rasmussen scale.Result:The blood loss, bone healing time and load-bearing exercise time in the observation group were significantly better than those in the control group(P<0.05).The excellent and good rate in the observation group(94.0%) was significantly higher than 70.0% in the control group(P<0.05).Conclusion:Compared with the traditional hollow nail internal fixation and locking plate internal fixation in the treatment of complex tibia plateau fractures,the trauma is less,which is more conducive to promoting postoperative bone healing,reliable fixation,in accordance with the principle of biological fixation.It is worth popularizing and applying in clinic.
