中医整脊治疗腰椎滑脱症临床研究

摘　　要：目的观察中医整脊治疗腰椎滑脱症(LS)的临床疗效并分析其作用机制。方法选择2018年3-6月在深圳市中医院推拿科就诊的60例LS患者,采用随机数字表法将患者分为观察组、对照组,各30例。观察组患者通过理筋松解、手法复位、四维牵引、功能锻炼等治疗,对照组予推拿治疗,2组治疗均进行2个疗程(10d为1个疗程),治疗前、治疗结束后1个月回访,观察2组患者日本骨科学会(JOA)评分、脊柱-骨盆矢状位参数指标。结果治疗结束后1个月回访,观察组总有效率明显高于对照组,JOA主观症状、临床体征日常活动、总分分值显著高于对照组,脊柱-骨盆矢状位参数骨盆倾斜角(PT)明显低于对照组,差异均有统计学意义(P<0.05或0.01)。结论中医整脊疗法相较于推拿对提高LS患者的疗效更佳,能显著提高JOA总分,更好地降低LS患者脊柱-骨盆矢状位参数的PT值,对LS具有良好的临床疗效。ObjectiveTo observe the clinical effect of chiropractic treatment of traditional Chinese medicine(TCM)for lumbar spondylolisthesis(LS)and analyze its mechanism.MethodsA total of 60 LS patients who were admitted to the Massage Department of Traditional Chinese Medicine Hospital of Shenzhen from March to June 2018 were selected,60 cases of LS patients were divided into observation group and control group with 30 cases each by random number table method.The experimental group was treated by tendon-soothing maneuver,manual reduction,four-dimensional traction,functional exercise and so on.The control group was treated with massage.Both groups received two courses of treatment(10 d for one course).The Japanese Orthopaedic Association(JOA)score and spinal-pelvic sagittal position parameters of the two groups were observed before the treatment and the time when patients were followed up one month after the treatment.ResultsOne month follow-up after treatment,the total effective rate of the experimental group was significantly higher than that of the control group,the subjective symptoms,clinical signs,daily activities were significantly higher than those in the control group,the pelvic tilt(PT)of the spinal-pelvic sagittal parameters was significantly lower than in the control group,the differences were statistically significant(P<0.05 or 0.01).ConclusionCompared with massage,Chinese chiropractic is more effective in improving the effect of LS patients.The total score of JOA significantly improved in the experimental group,and the PT scores of spinal-pelvic sagittal parameters got lower in LS patients.Chinese chiropractic has a favourable clinical effect on LS.
