电针足少阳头项部经穴治疗偏头痛临床观察

摘　　要：目的:观察电针足少阳头项部经穴治疗偏头痛的临床疗效,探讨头项部电针治疗偏头痛的作用机制,为临床治疗偏头痛提供新的思路。方法:将符合偏头痛诊断标准的64例病例采用随机数字表法分为治疗组与对照组,治疗组采用电针足少阳头项部经穴治疗,对照组选取足少阳经常规穴位针刺治疗,每次治疗30min,每周6次,共治疗4周,治疗结束后电话随访半年确定远期疗效。通过观察两组偏头痛患者治疗前后疼痛程度、疼痛持续时间、疼痛的伴随症状、治疗期间的发作次数以及日常生活能力比较两组的疗效。结果:治疗疗程结束后治疗组总有效率为91.18%,对照组总有效率为67.64%,治疗组明显高于对照组,两组疗效比较差异有统计学意义(P<0.05);治疗后两组VAS评分、MPQ评分、MIDAS评分比较,治疗组优于对照组(P<0.05)。结论:电针足少阳头项部经穴和常规穴位针刺治疗对偏头痛均有效,电针足少阳头项部经穴治疗效果更明显,值得临床推广应用。但针刺治疗可能存在治疗后时间太长而疗效降低的情况。Objective: To observe the clinical efficacy of electro-needling head and neck points of Gallbladder Meridian of Foot-Shaoyang in the treatment of migraine, and to explore its action mechanism thus to provide new ideas for clinical treatment of migraine. Methods: 61 patients who met the diagnostic criteria for migraine were randomly divided into the treatment group and the control group. The treatment group was treated with electro- needling head and neck points of Gallbladder Meridian of Foot- Shaoyang;whereas, the control group was treated with needling routine points of Gallbladder Meridian of Foot-Shaoyang. The treatment was 30 minutes each time, and 6 times a week for 4 weeks. After the treatment, telephone follow-up was carried out for half a year to determine the long-term efficacy. The efficacies of the two groups were compared by observing the degree of pain, the duration of pain, the accompanying symptoms, the number of episodes during the treatment and the ability of daily living before and after the treatment in the two groups. Results: After the treatment, the total effective rate of the treatment group was 91.18%, which was significantly higher than 67.64% of the control group( P <0.05). In terms of VAS score, MPQ score and MIDAS score afcer the treatment, the treatment group was better than the control group( P <0.05). Conclusion: Electro-needling head and neck points and routine needling common points of Gallbladder Meridian of Foot-Shaoyang are both effective for migraine. The electro-acupuncture is more effective and worthy of clinical application. Acupuncture treatment may have a decreased efficacy after a long period of time.
