摘 要:目的探讨两种抗菌素对杀白细胞素基因阳性[PVL(+)]耐甲氧西林金黄色葡萄球菌(MRSA)感染的治疗效应。方法在兔背部埋入一个中空、多孔的高尔夫练习球建立兔组织笼模型后,将16只新西兰雌兔随机均分为A、B、C和D四组,分别予以注射PVL(+)MRSA、PVL(-)MRSA、ATCC43300和ATCC25923菌株。当组织笼内细菌浓度达到1×10~8 CFU/ml时,每组随机选2只兔子予万古霉素治疗(A1、B1、C1、D1组),另2只予利奈唑胺治疗(A2、B2、C2、D2组)。每只兔子治疗前、治疗期间每次给药前及治疗达药物浓度稳态后1~3 d测定组织笼内细菌浓度。结果治疗前、后,A2组细菌浓度低于A1组(F=10560.216,P<0.01),B2组细菌浓度低于B1组(F=29.105,P<0.05),且随治疗时间的延长,A2组细菌浓度下降的幅度大于A1组(F=2052.739,P<0.01),B2组细菌浓度下降的幅度大于B1组(F=31.743,P<0.05)。而C1组与C2组(F=1.840,P>0.05)、D1组与D2组(F=1.468,P>0.05)间细菌浓度差异无统计学意义。结论利奈唑胺抗MRSA作用优于万古霉素,对PVL(+)MRSA导致的皮下软组织感染效果更明显。Objective To explore the therapeutic efficiency of two antibiotics in rabbits infected with Panton-Valentine leukocidin-positive [PVL(+)] methicillin-resistant Staphylococcus aureus(MRSA).Methods After established the rabbit tissue cage model,16 New Zealand female rabbits were equally randomized into four groups,which were respectively injected with PVL(+)MRSA(group A),PVL(-)MRSA(group B),ATCC43300(group C) and ATCC25923(group D).When the concentration of bacteria in the tissue cage reached to a level of 1×10~8 CFU/ml,2 rabbits in each group were treated with vancomycin(groups of A1,B1,C1 and D1) and other 2 rabbits were treated with linezolid(groups of A2,B2,C2 and D2).The bacterial concentration in each rabbit tissue cage was determined before treatment,during treatment and within 3 days after drug concentration homeostasis.Results The bacterial concentration in group A2 was lower than that in group A1(F=10560.216,P<0.01),while that in group B2 was lower than that in group B1(F=29.105,P<0.05) during treatment.With the prolongation of treatment time,the decrease of bacterial concentration in group A2 was greater than that in group A1(F=2052.739,P<0.01), and that in group B2 was greater than that in group B1(F=31.743,P<0.05).There was no significant difference in bacterial concentration between group C1 and group C2(F=1.840, P>0.05), so did between groups of D1 and D2(F=1.468, P>0.05).Conclusion Linazolamide has better anti-MRSA efficiency than vancomycin,especially in the subcutaneous soft tissue infection caused by PVL(+)MRSA.
