摘 要:目的利用扩散加权成像表观扩散系数(DWI-ADC)和光学成像观察小鼠胃癌移植瘤模型的动态变化,评价胃癌生物学行为的演变。材料与方法体外培养胃癌 BGC-823 细胞株悬液,建立 Balb/c 裸小鼠皮下移植瘤与原位移植瘤动物模型各 5 只。分别于第 5、9、13、17、21、25、28 天和第 7、10、14、21、28、35、42 天记录 T1WI 和 T2WI 序列中皮下移植瘤与原位移植瘤的大小变化,测量 DWI-ADC 值的动态变化。对 BGC-823 细胞进行荧光质粒转染,对移植瘤进行动态光学成像,计算平均光子数。以末期 HE 染色的组织病理为依据,比较 DWI-ADC 及光学成像反映肿瘤变化特征的差异。结果皮下移植瘤与原位移植瘤在观察期间均呈渐进性增大。皮下移植瘤始终呈膨胀性生长,原位移植瘤于第 28 天后呈浸润性生长。两种移植瘤平均 ADC值均表现为先下降,继而稳定,最后略升高;平均光子数均呈先升高,后下降趋势。两种移植瘤 ADC 值曲线转折点的出现均早于各自光学曲线。原位移植瘤的微小转移灶在 MRI 与光学成像中均未显示。结论 DWI-ADC 与生物发光成像均可于早期反映肿瘤生物学行为的变化,提示肿瘤内活性成分的变化;且 DWI-ADC 的敏感性高于生物发光成像。Purpose To observe the dynamic changes of gastric cancer transplantation tumors in mice using diffusion-weighted imaging with apparent diffusion coefficient (DWI-ADC) and bioluminescence imaging, and to evaluate the evolution of biological behavior of gastric cancer. Materials and Methods The suspension of gastric cancer BGC-823 cell line was cultured in vitro, 5 Balb/c nude mice subcutaneous transplantation tumors and 5 orthotopic transplantation tumors were established respectively. The changes to the sizes of subcutaneous transplantation tumors and orthotopic transplantation tumors in T1WI and T2WI sequences were recorded at day 5, 9, 13, 17, 21, 25 and 28, day 7, 10, 14, 21, 28, 35 and 42, respectively. The dynamic changes to DWI-ADC values were measured. The BGC-823 cells were transfected with luciferase plasmid, the transplantation tumors were subjected to dynamic bioluminescent imaging and the average photon number was calculated. Based on the histopathology of late HE staining, the differences in the tumor change characteristics reflected by DWI-ADC and bioluminescence imaging were compared. Results Subcutaneous transplantation tumors and orthotopic transplantation tumors increased gradually during the observation period. Subcutaneous transplantation tumors always showed expansive growth, and orthotopic tumors experienced invasive growth after 28 days. The average ADC values of both transplantation tumors decreased first, then remained stable, and finally increased slightly;the average number of photons increased first and then decreased. The knee of the curve of ADC values of both transplantation tumors occurred earlier than the respective bioluminescence curves. Micrometastases of orthotopic transplantation tumor were not shown in both magnetic resonance and bioluminescence imaging. Conclusion Both DWI-ADC and bioluminescence imaging can reflect the changes to biological behavior of gastric cancer in the early stage, suggesting the changes to active components in tumors, and the sensitivity of DWI-ADC i
