摘  要:目的:以血小板显微镜法(PLT-M)为参照,比较血小板电阻抗法(PLT-I)、光学法(PLT-O)及荧光法(PLT-F)计数异常血小板的准确性。方法:收集EDTA抗凝全血标本,经血常规检测后,低值血小板组、大血小板组和小红细胞组各收集50例标本,分别采用PLT-I、PLT-O、PLT-F和PLT-M计数血小板,且以PLT-M作为衡量标准进行比较分析。结果:与PLT-I、PLT-O、PLT-M方法相比,PLT-F法的重复性最好、精确度最高;在低值血小板和大血小板组,PLT-I、PLT-O、PLT-F与PLT-M的相对偏差和绝对偏差均较小,各种方法相关系数R值均大于0.99(均P<0.05);而在小红细胞组中,PLT-I、PLT-O与PLT-M的相对偏差和绝对偏差均较大,R值均小于0.98(均P>0.05),PLT-F与PLT-M的相对偏差和绝对偏差较小,R值大于0.99(P<0.05)。结论:临床常规标本血小板计数可以采用PLT-I和PLT-O方法检测,但当出现小红细胞时,有必要采用PLT-F方法进行复检,以提高血小板计数的准确性。Objective: To analyze the accuracy of three methods when compared with platelet microscopy method (PLT-M) including platelet impedance method (PLT-I), platelet optical method (PLT-O) and platelet fluorescence method (PLT-F) for abnormal platelet count.Methods: Whole blood samples of EDTA anticoagulation were collected for routine blood test.There were 50 cases of specimens in the low value platelet, large platelet and small red blood cell group, respectively.Platelets were counted by PLT-I, PLT-O , PLT-F and PLT-M.PLT-M was used as the measurement standard for comparative analysis.Results: Compared with PLT-I, PLT-O, and PLT-M methods, PLT-F performed with the best repeatability and the highest accuracy.In the low value platelet and large platelet groups, the relative deviations and absolute deviations of PLT-I, PLT-O and PLT-F from PLT-M were all small, with R values more than 0.99(All P <0.05).In the small red blood cell group, the relative deviation and absolute deviation of PLT-I and PLT-O from PLT-M were both large, with R values less than 0.98 (All P >0.05).However, The relative deviation and absolute deviation of PLT-F from PLT-M were both small, with R value greater than 0.99( P <0.05 ).Conclusion: Platelet counts of clinical routine specimens can be detected by PLT-I and PLT-O methods.It is necessary to use PLT-F method for re-examination when small red blood cells appear to improve the accuracy of platelet count.
