两种术式在剖宫产瘢痕妊娠治疗中的对比研究

摘　　要：目的通过对比B超引导与宫腔镜引导这两种清宫治疗手术方式在剖宫产瘢痕妊娠治疗中的治疗效果,为该类疾病的治疗提供重要指导依据.方法收集2015.01-2018.06期间于西安市第四医院妇产科收治的80例剖宫产瘢痕妊娠患者的病例资料进行回顾性研究.依据治疗方法的不同将全部患者分成两组(n=40):试验组(药物+宫腔镜引导下清宫治疗)和对照组(药物+B超引导下清宫治疗).结果在剖宫产瘢痕妊娠治疗方面,两种手术方式均有疗效.进一步分析发现,试验组手术效果显著优于对照组,具体表现为:术中出血含量少(P<0.05),术后绒毛膜促性腺激素(HCG)含量低(P<0.05),术后月经恢复时间短(P<0.05).结论在剖宫产瘢痕妊娠治疗中,宫腔镜下清宫治疗比B超引导下清宫治疗的临床效果更好,值得推广应用.Objective To compare the curative effects of ultrasound - guided uterine curettage and hysteroscopic uterine curettage on cesarean scar pregnancy. Methods A retrospective analysis was made to the clinical data of 80 cases with cesarean scar pregnancy hospitalized during the period from Jan.,2015 to June,2018;the cases were divided into 2 groups: experiment group (drug + ultrasound - guided uterine curettage) and control group (drug + hystero-scopic uterine curettage),40 in each. Results The 2 surgical methods were effective in the treatment of cesarean scar pregnancy while the effect in experiment group was superior to that in control group with smaller blood loss in operation,lower HCG level after operation and shorter recovery time for postoperative menstrual (P <0.05). Conclusions Hysteroscopic uterine curettage is of better effect in the treatment of cesarean scar pregnancy than ultrasound - guided uterine curettage and it is worthy of wider clinical application.
