摘  要:目的分析贝那普利联合自体脾细胞治疗对肾小球肾炎(GN)大鼠免疫指标、肾功能和肾小球硬化的影响。方法取30只清洁级SD大鼠随机分为对照组(给予生理盐水进行灌胃)、单药组(给予贝那普利水溶液进行灌胃)与联合组(贝那普利水溶液灌胃+输注自体脾细胞),三组各10只。比较三组大鼠疗后4周血肌酐、血尿素氮和CD4、CD8阳性细胞百分比的变化情况及肾小球硬化评分的差异。结果治疗4周后,联合组血肌酐和血尿素氮含量较单药组均显著下降(P<0.05),且与对照组比较,差异无统计学意义(P>0.05)。单药组、联合组CD4和CD8阳性细胞百分率较对照组均显著升高,同时联合组较单药组显著升高(P<0.05)。治疗后4周,联合组肾小球硬化评分较单药组显著下降,而单药组评分较对照组显著升高(P<0.05)。结论贝那普利联合自体脾细胞治疗可有效改善GN大鼠肾功能,提高免疫功能,可有效阻滞肾小管损伤,减轻肾小球硬化,因此具有良好的临床应用前景。Objective To analyze the effects of benazepril combined with autologous spleen cells on immune indexes,renal function and glomerulosclerosis in rats with glomerulonephritis (GN). Methods Thirty clean SD rats were randomly divided into control group (given normal saline for gastric lavage),single drug group (given benazepril solution for gastric lavage) and combined group (benazepril solution for gastric lavage + autologous splenocyte infusion),with 10 cases in each group. The changes of serum creatinine,blood urea nitrogen,percentage of CD4 and CD8 positive cells and the difference of glomerulosclerosis score in the three groups were compared 4 weeks after treatment. Results The serum creatinine and urea nitrogen levels in the combined group were significantly lower than those in the single drug group ( P <0.05),and there was no significant difference compared with the control group ( P >0.05). The percentage of CD4 and CD8 positive cells in single drug group and combination group was significantly higher than that in control group,and the percentage of CD4 and CD8 positive cells in combination group was significantly higher than that in single drug group ( P <0.05). Four weeks after treatment,the glomerulosclerosis score of the combined group was significantly lower than that of the single drug group,while the score of the single drug group was significantly higher than that of the control group ( P <0.05). Conclusion Benazepril combined with autologous spleen cells can effectively improve the renal function and immune function of GN rats,effectively block renal tubular injury and reduce glomerulosclerosis,so it has a good clinical application prospect.
