摘　　要：目的探讨幽门螺杆菌(Hp)感染对非酒精性脂肪肝(NAFLD)患者血管内皮功能及颈动脉粥样硬化的影响。方法将临床确诊的248例NAFLD患者依据13C尿素呼气试验(13C-UBT)检测结果分为感染组(128例)、非感染组(120例),对两组血清一氧化氮(NO)、内皮素-1(ET-1)、同型半胱氨酸(Hcy)、高敏C-反应蛋白(hs-CRP)、血脂、空腹血糖(Bs)、肱动脉内皮舒张功能、颈动脉内中膜厚度(CIMT)、颈动脉斑块(CP)检测比较。结果与非感染组比较,感染组CIMT、CP检出率、血清ET-1、三酰甘油(TG)、总胆固醇(CH)、低密度脂蛋白胆固醇(LDL-C)及Bs、Hcy、hs-CRP显著升高,血清N0、肱动脉内皮依赖性舒张功能(FMD)、非内皮依赖性舒张功能(NMD)显著降低,两组比较差异有统计学意义(P<0.05)。结论Hp感染可能通过影响机体脂质代谢、引发炎症反应,促进NAFLD患者内皮功能的损伤和动脉粥样硬化的发生发展。Objective To investigate the impact of Helicobacter pylori(Hp)infection on vascular endothelial function and carotid atherosclerosis in patients with nonalcoholic fatty liver disease(NAFLD).Methods According to the results of 13C-UBT,a total of 248 patients with NAFLD were divided into Hp infection group(128 cases)and non-Hp infection group(120 cases).The serum nitric oxide(NO),endothelin-1(ET-1),serum Hcy,high-sensitivity C reactive protein(hs-CRP),serum lipid parameters,blood glucose(Bs),brachial artery endothelial function,carotid artery intima-media thickness(CIMT)and carotid plaquewere(CP)compared between the two groups.Results Compared with the non-Hp infection group,CIMT,the detection rate of CP,ET-1,TG,CH,LDL-C,Bs,Hcy,hs-CRP in the Hp infection group were significantly higher,while serum NO,brachial artery endothelium-dependent diastolic function(FMD)and non-endothelium-dependent diastolic function(NMD)were significantly lower(P<0.05).Conclusion Hp infection may aggravates the endothelial dysfunction and atherosclerosis in NAFLD patients by influencing lipid metabolism and causing inflammation.
