摘  要:目的探讨慢性阻塞性肺疾病(COPD)患者外周血单个核细胞微小RNA(miR)-25的表达水平及其意义。方法纳入于我院就诊的COPD患者127例,其中稳定期患者65例作为稳定期组,急性加重期患者62例作为急性加重期组,同时选取50例健康者作为对照组。检测所有受试者的肺功能和外周血单个核细胞miR-25表达水平并进行比较。采用Pearson相关分析评估COPD患者外周血单个核细胞miR-25与肺功能指标的相关性。采用受试者工作特征(ROC)曲线评估外周血单个核细胞miR-25诊断AECOPD的价值。结果3组患者第1秒用力呼气容积(FEV,)、用力肺活量(FVC)及FEV1/FVC比较,差异均有统计学意义(P<0.05)。急性加重期组和稳定期组FEV1、FVC、FEV1/FVC及外周血单个核细胞miR-25表达水平低于对照组(P<0.05);急性加重期组FEV1、FVC、FEV/FVC外周血单个核细胞miR-25表达水平低于稳定期组(P<0.05).Pearson相关分析结果显示,急性加重期组患者外周血单个核细胞miR-25与FEV]呈正相关(P<0.05),而与FVC、FEV1/FVC无相关性(P>0.05)。稳定期组患者外周血单个核细胞miR-25与FEV1、FVC、FEV1/FVC均呈正相关(P<0.05)。ROC曲线分析结果显示,外周血单个核细胞miR-25诊断COPD急性加重期(AECOPD)的ROC曲线下面积(AUC)为0.810(95%CI0.722~0.898),敏感度和特异度分别为91.2%和71.4%,约登指数为0.63。结论COPD患者外周血单个核细胞miR-25水平低于健康者,且其与肺功能指标相关,有望成为AECOPD患者早期诊断的参考指标。Objective To explore the expression and significance of microRNA ( miR)-25 in peripheral blood mononuclear of patients with chronic obstructive pulmonary disease( COPD). Methods A total of 127 patients with COPD in our hospital were included, and 65 patients in stable period were selected as stable period group,62 patients in acute exacerbation period were as acute exacerbation period group. At the same time,50 healthy volunteers were selected as control group. Lung function and expresion level of miR-25 in peripheral blood mononuclear cells of all subjects were detected and compared. The correlation between miR-25 in peripheral blood mononuclear cells and lung function were analyzed by Pearson correlation analysis in patients with COPD. The value of miR-25 in peripheral blood mononuclear cells in diagnosis of AECOPD was analyzed by receiver operating characteristic curve ( ROC) curve. Results Forced expiratory volume in one second ( FEV1 ), forced vital capacity ( FVC) and FEV\/FVC between three groups were significantly different(P <0. 05). FEV(,FVC,FEV〔/FVC and expresion level of miR-25 in peripheral blood mononuclear cells in stable period group and acute exacerbation period group were all lower than those in control group(P <0. 05). FEV1,FVC,FEV1/FVC and expresion level of miR-25 in peripheral blood mononuclear cells in acute exacerbation period group was lower than that in control group( P <0. 05). Results of Pearson correlation analysis showed that miR-25 in peripheral blood mononuclear cells was positively correlated with FEV1( P < 0. 05 ), and was not correalted with FVC, FEV1/FVC in patients of acute exacerbation period group ( P > 0. 05 ). MiR-25 in peripheral blood mononuclear cells was positively correlated with FEV1 , FVC and FEV1/FVC in patients of stable period group( P < 0. 05 ). Results of ROC curve showed that ROC curve area of MiR-25 in peripheral blood mononuclear cells in diagnosis of acute exacerbation of COPD ( AECOPD) was 0. 810 ( 95% CI 0. 722-0. 898),and the sensitivity and specificit
