降钙素原在急诊泌尿系感染并脓毒症患者中的应用价值

摘　　要：目的:探讨降钙素原(Proealcitonin,PCT)在急诊泌尿系感染脓毒症患者中的应用价值。方法:回顾 2017 年 1 月 1 日至 2018 年 12 月 31 日我院急诊科病区收治的 69 例泌尿系感染患者(男性 24 例,女性 45 例),将其分为脓毒症组(n=24)和非脓毒症组(n=45)。根据是否梗阻分为梗阻组(n=35)与非梗阻组(n=34),再将梗阻组分为手术组(n=12)和非手术组(n=23),观察每组患者 PCT 与血白细胞计数(White blood count,WBC)。结果:脓毒症组 PCT、WBC 较非脓毒症组明显升高(P<0.05)。梗阻组 PCT 较非梗阻组明显升高(P<0.05),WBC没有显著性差异(P>0.05)。手术组 PCT、WBC 较非手术组明显升高(P<0.05)。结论:降钙素原是泌尿系严重感染较为敏感及准确的一项检验指标,合并结石尤其是并发脓毒症患者中明显升高,因此早期可根据患者 PCT结果,对结石高危患者进行筛查,并早期进行干预,从而根据 PCT 动态结果指导治疗。Objective:To investigate the value of proealcitonin(PCT)of urinary tract infection complicated with sepsis. Methods:69 cases(24 males,45 females)of urethral infection patients in our department of emergency from January 1,2017 to December 31,2018 were retrospective analyzed,which were divided into sepsis group(n=24)and non-sepsis group(n=45). According to whether or not obstruction,patients were divided into obstruction group(n=35) and non-obstruction group(n=34),then patients in the obstruction group were divided into operation group(n=12)and non-operation group(n=23). PCT and white blood cell count were observed in each group. Results:The PCT and WBC in the sepsis group were significantly higher than those in non - sepsis group(P<0.05). PCT in the obstruction group increased significantly(P<0.05),however,WBC had no significant difference in the obstructive group(P>0.05). PCT and WBC in the operation group w were significantly higher in the operation group than those in the non-operation group (P<0.05). Conclusion:Procalcitonin is a sensitive and accurate test index for severe urinary tract infections,and significantly increases in patients with urinary calculi,especially in sepsis. Therefore,early screening of high-risk patients in urinary tract infection and early intervention can be carried out according to the results of PCT,so as to guide the treatment according to the dynamic results of PCT.
