基于电子管理系统的2型糖尿病临床路径变异分析

摘　　要：目的分析基于电子管理系统2型糖尿病临床路径开展过程中产生的变异环节及变异原因,提出可行性整改措施,初步建立医院临床路径的变异管理模式,促进临床路径管理的顺利实施。方法收集医院电子管理系统2015年1月—2016年12月实施临床路径管理的2型糖尿病纸质病历与电子病历资料进行回顾性分析,统计不同变异类型的发生率,并实施针对性整改措施。结果2型糖尿病临床路径总变异率高达48.79%,其中可控变异561例,占39.07%;不可控变异875例,占60.93%。结论通过提高患者依从性,明确医务人员责任,完善绩效考核办法及优化电子管理系统,可有效降低2型糖尿病临床路径变异的发生率。Objective This study was to analyze the causes of variation in the clinical pathway of type 2 diabetes,and proposed feasible rectification measures to promote the smooth implementation of the clinical pathway by effective methods. Methods Retrospective analysis was performed on type 2 diabetes,which was administered by clinical pathway management in the electronic management system from January 2015 to December 2016,and the incidence of different variation types was analyzed by SPSS 19.0. Results The total variation rate of clinical pathway of type 2 diabetes in our hospital was up to 48.79 %,among which controllable variation had 561 cases,accounting for 39.07 %,and uncontrollable variation had 875 cases, accounting for 60.93 %.Conclusion It can effectively reduce the incidence of variation of clinical pathway in our hospital by improving patient compliance,making a clear responsibility for health care workers,improving performance assessment methods and optimizing electronic management system.
