摘 要:目的:探讨快速冷冻切片、DNA定量分析及常规石蜡诊断在甲状腺疾病中的应用价值。方法:选择本院手术中获取的甲状腺标本258例,采用快速冷冻切片、DNA定量分析和常规石蜡诊断3种方法进行病变性质的判断,以常规石蜡诊断结果为金标准。结果:3种方法诊断结果的阳性率差异无统计学意义(P=0.357);快速冷冻切片诊断的准确度为98.06%(253/258),DNA定量分析的灵敏度为94.38%(84/89),特异度为97.63%(165/169),阳性预测值为95.45%(84/88),阴性预测值为97.06%(165/170),准确度为96.51%(249/258)。结论:在术中甲状腺组织快速病理诊断中,DNA定量分析与快速冷冻切片均可以获得较高的准确度,为术中快速病理诊断提供了新的方法。Objective:To explore the application value of rapid freezing section, DNA quantitative analysis and conventional paraffin diagnosis in thyroid diseases.Methods:A total of 258 thyroid specimens obtained during the operation of our hospital were selected, and three methods of rapid freezing section, quantitative DNA analysis and conventional paraffin diagnosis were used to determine the pathological characteristics. Conventional paraffin diagnosis results were the gold standard.Results:There was no statistically significant difference in the positive rates among the three methods ( P =0.357). The diagnostic accuracy of rapid freezing section was 98.06 %(253/258). The results of quantitative DNA analysis showed the sensitivity with 97.63 %(165/169), positive predictive value with 95.45 %(84/88), negative predictive value with 97.06 %(165/170), accuracy with 96.51 %(249/258).Conclusion:In the rapid pathological diagnosis of intraoperative thyroid tissue, both quantitative DNA analysis and rapid frozen section can obtain high accuracy, providing a new method for rapid intraoperative pathological diagnosis.
