摘 要:目的通过参附注射液对创伤性急性肺损伤血管内皮细胞功能的研究,期望为创伤性急性肺损伤的治疗提供新的途径。方法将该院2016年6月—2018年6月收治的符合纳入标准的创伤性急性肺损伤患者80例,随机分为观察组(n=40)与对照组(n=40)。观察组为西药常规治疗加参附注射液组,给予5%葡萄糖注射液250mL+参附注射液50mL静滴,1次/d;对照组为西药常规治疗组。以上两组治疗均以7d为1个疗程,将治疗前后患者呼吸频率及实验室检查指标做观察比较。结果与对照组相比,观察组治疗后呼吸频率(22.50±3.56)次/min,优于对照组(t=2.27,P=0.029);PO2、PaO2/FiO2指标数据分别为(81.92±3.33)mmHg、(300.51±21.44),均优于对照组(t=2.144,2.328,P=0.038,0.025);ICAM-1、ET-1及NO分别为(12.98±2.41)ug/mL、(75.39±10.51)ng/mL、(28.18±5.7)ng/mL,均显著优于对照组(t=7.343,5.174,5.628,P=0.000);治疗后观察组总有效率77.5%明显高于对照组55.0%,两组比较,差异有统计学意义(χ^2=4.53,P<0.05)。结论参附注射液能明显改善创伤性急性肺损伤患者的呼吸频率,改善氧合指数,降低ICAM-l、ET-1、NO的水平,改善创伤性急性肺损伤患者的预后。Objective Study on endothelial cell function of traumatic acute lung injury with Shenfu injection, expectations for the acute lung injury prevention and control to provide a new way. Methods Patients with traumatic acute lung injury admitted to our hospital from June 2016 to June 2018 were randomly divided into observation group (n=40) and control group (n=40). The observation group received the Western medicine routine treatment plus ginseng injection group, with 5% glucose injection 250ml + ginseng injection 50ml intravenous drip, once a day. The control group received the Western medicine routine treatment group. In the above two groups, the respiratory rate and laboratory examination indexes of the patients were observed and compared in 7 days. Results The respiratory frequency (22.50±3.56) per minute after treatment in the observation group was better than that in the control group(t=2.27, P=0.029). The data for PO2 and PaO2/FiO2 were (81.92±3.33) mmHg and (300.51±21.44), respectively, which were better than those in the control group(t=2.144,2.328, P=0.038,0.025). The ICAM-1, ET-1 and NO were (12.98±2.41)ug/mL,(75.39±10.51)ng/mL(28.18±5.7)ng/mL, respectively, which were significantly better than those in the control group (t=7.343, 5.174, 5.628, P=0.000). The total effective rate was 77.5% in post-treatment observation group, which was significantly higher than that in control group (55.0%). There was statistical difference between the two groups(χ^2=4.53, P<0.05). Conclusion Shenfu injection can significantly improve respiratory rate and oxygenation index, reduce the levels of ICAM-1, ET-1, NO, and improve the prognosis of patients with traumatic acute lung injury.
