抗酸染色阳性质控方法的建立及临床应用

摘　　要：目的评价卡介苗作为抗酸染色阳性质控方法的可行性。方法首先,从药剂科领取上海生物制品研究所有限责任公司生产的卡介苗冻干粉末,按标示量复溶后制备20张涂片,贴好编号标签。紫外线照射1小时,自然干燥并加热固定放置在染色架上。按照抗酸染色标准操作程序执行染色,然后在OLYMPUSBX40型光学显微镜100×油镜下镜检,评估卡介苗制备的20张片染色效果并与结核分枝杆菌H37RaATCC25177染色效果比较。其次,由10名技术人员分别对卡介苗制片进行抗酸染色和镜检来评估重复性和稳定性。最后,回顾5年内用卡介苗制备的619质控品结果。结果卡介苗制备的20张抗酸涂片油镜镜检均发现抗酸杆菌,染色效果与结核分枝杆菌H37RaATCC25177抗酸染色效果一致且重复性和稳定性均较好。5年内用卡介苗制备的619份质控品结果均为阳性。结论实验室可采用卡介苗作为抗酸染色阳性质控的方法,且易于获得、价格低廉。Objective valuate the feasibility of Bacillus Calmette-Guerin (BCG) vaccine used as the positive control for acid-fast stain test.Methods Firstly,BCG vaccine from Pharmacy produced by Shanghai institute of biological products limited company,was disolved based on instructions and 20 smears prepared with labels.Ultraviolet irradiation for 1 hour,natural drying and fixing by heating,then smears were placed on the staining shelf to get acid-fast stain according to standard operating procedure.Stained smears were observed under OLYMPUS BX40 100× optical microscope to evaluate the staining results,which were compared with the Acid-fast stain result of M.TB H37Ra ATCC25177.Secondly,10 lab technologists performed Acid-fast stains with smears prepared from BCG vacine in order to evaluate reproducibility and stability.Lastly,review the 619 QC results prepared by BCG vaccine in the past 5 years.Results Acid-fast bacilli were found in all 20 smears made from BCG Vaccine with good reproducibility and stability,and the staining results were similar to that of M.TB H37Ra ATCC25177.The 619 QC results prepared by BCG vaccine in the past 5 years were all positive.Conclusion BCG can be used as the positive control of Acid-fast stain and it is easily available and cost effective.
