根本原因分析法在血站医疗废物管理中的应用

摘　　要：目的尝试应用根本原因分析法,消除血站流动采血点医疗废物管理中的隐患,优化管理流程。方法对武汉血液中心2017年1-5月份所涉及的658张《各科室医疗废物移交收集记录》进行统计分析,重点关注流动采血点交接记录,确认流动采血点医疗废物管理中问题存在的根本原因,制定纠正预防措施,并执行改进方案。结果针对流动采血点医疗废物实际交接流程与制度规范不符、暂存间设施陈旧、监管制度不严等问题,采用二联单交接模式的应用、增加门禁管理系统、加强监管及安全防护培训等措施改进。收集人员与流动点实现了医疗废弃物点对点交接,流动采血点医疗废弃物管理更加规范,中心医疗废物交接记录实际记录登记地点合格率从21.2%达到100.0%全覆盖,医疗废物重要信息正确填写率由34.8%提高到93.4%。结论根本原因分析法应用于血站医疗废物管理,可以优化流动采血点医疗废物交接流程,改良效果明显。Objective root cause analysis was applied to eliminate hidden dangers in medical wastes management of mobile blood collection points,thus optimizing the management flow.Methods The department transfer records of medical waste collection of Wuhan Blood Center ( n =658,from January to May in 2017) was collected and statistical analyzed.We focused on the records from mobile blood collection points and figured out the issues and reasons in medical wastes management,then corrective and preventive measures were formulated and an improvement scheme was proposed.Results To all problems we encountered such as discrepancy between actual handover process and system spec,older facilities in temporary storage facility and lax regulation system,we adopt the two duplicate delivery mode of application,increased the access control management system,and strengthened the supervision and training of safety protection measures.Thus,point-to-point transfer of medical wastes from trash collectors to mobile blood collection points was realized,mobile blood point medical wastes management was more standardized,qualified rate of actual registration site of medical wastes transfer records was increased from 21.2% to 100.0%,correct rate of medical wastes′ important information in records was promoted from 34.8% to 93.4%.Conclusion Our results showed that root cause analysis was suitable for medical wastes management of blood stations,it can optimize and improve medical wastes handover process obviously.
