五苓散治疗痰湿内阻型非酒精性脂肪性肝病62例

摘　　要：目的:观察五苓散治疗痰湿内阻型非酒精性脂肪性肝病的临床疗效。方法:将124例非酒精性脂肪性肝病患者按照随机数字表法分为治疗组和对照组,每组各62例。治疗组给予五苓散加减治疗,对照组给予多烯磷脂酰胆碱胶囊。检测两组患者治疗前后丙氨酸氨基转移酶(alanine transaminase,ALT)、三酰甘油(three acyl glycerin,TG)、血清总胆固醇(total cholesterol,TC)水平。比较两组患者治疗前后肝/脾CT比值、中医证候积分及单项临床症状积分改善情况。结果:两组患者治疗后ALT、TC、TG水平及中医证候积分低于治疗前,且治疗组低于对照组,差异具有统计学意义(P <0. 05)。两组患者治疗后肝/脾CT比值高于治疗前,且治疗组高于对照组,差异具有统计学意义(P <0. 05)。治疗组治疗后右胁不适、周身困重、脘腹胀满、倦怠乏力等单项症状积分低于治疗前,对照组治疗后脘腹胀满、倦怠乏力等积分低于治疗前(P <0. 05),对照组治疗后右胁不适、周身困重2个单项症状积分与治疗前比较,差异无统计学意义(P> 0. 05)。治疗组治疗后各单项症状积分均低于对照组,差异均有统计学意义(P <0. 05)。结论:五苓散治疗痰湿内阻型非酒精性脂肪性肝病疗效确切,可改善患者的肝功能及临床症状。Objective: To observe the clinical curative effect of Five Substances Powder with Poria on non-alcoholic fatty liver disease with internal obstruction of phlegm and dampness. Methods: A total of 124 patients with nonalcoholic fatty liver disease were randomly divided into the treatment group and the control group,with 62 cases in each group. The treatment group was treated with Five Substances Powder with Poria and the control group was treated with Polyene Phosphatidylcholine Capsules. Alanine aminotransferase( ALT),triacyl glycerin( TG) and total cholesterol( TC) were measured before and after the treatment in both groups. The improvement of liver/spleen CT ratio,TCM syndrome score and single TCM symptom score before and after treatment were compared between the two groups. Results: After the treatment,the levels of ALT,TC,TG and the scores of TCM syndromes in the two groups were lower than those in the control group,and all the differences were statistically significant( P < 0. 05). After the treatment,the ratio of liver/spleen CT in each of the two groups was higher than that before the treatment,and that in the treatment group was higher than that in the control group,the difference was statistically significant( P < 0. 05). After the treatment,the scores of single symptoms such as right hypochondriac discomfort,body sleepiness,abdominal distention,fatigue and fatigue in the treatment group were lower than those before the treatment,while those in the control group were lower than those before treatment( P < 0. 05). There was no significant difference in the scores of two single symptoms of right hypochondriac discomfort and body sleepiness in the control group after the treatment compared with those before the treatment( P > 0. 05). After the treatment,the scores of each symptom in the treatment group were lower than those in the control group,and all the differences were statistically significant( P < 0. 05).
