摘 要:目的:观察肺炎支原体肺炎(MPP)患儿胸片改变为节段性肺炎或支气管肺炎时的年龄、发热时间、外周血WBC、血清C反应蛋白(CRP)及血清肺炎支原体抗体(MP-IgG)滴度变化情况。方法:我院收治的219例MPP患儿根据胸片改变情况分为节段性肺炎组和支气管肺炎组,检测两组患儿外周血WBC、CRP及MP-IgG滴度指标。结果:219例MPP患儿中,节段性肺炎组65例(29.6%),平均年龄(93.98±14.57)月,支气管肺炎组154例(70.4%),平均年龄(54.73±16.43)月(P<0.05);入组时平均发热时间节段性肺炎组(3.52±1.48) d,支气管肺炎组(2.51±0.99) d(P<0.05);节段性肺炎组血清MP-IgG≥1∶640的例数多于支气管肺炎组(P<0.05);节段性肺炎组血清CRP高于支气管肺炎组(P<0.05)。结论:MP感染后肺部炎症表现为节段性肺炎的患儿年龄偏大、发热时间较长、血CRP水平和MP-IgG滴度更高,提示患儿全身炎症反应强烈,年长儿童MP感染后易发生严重肺部病变。Objective: To observe the changes of age,fever time,peripheral blood WBC,serum C-reactive protein( CRP) and serum Mycoplasma pneumoniae antibody titer( MP-IgG) in children with Mycoplasma pneumoniae pneumonia( MPP) when the chest radiograph was changed to segmental pneumonia or bronchopneumonia.Methods: A total of 219 children with MPP were divided into the segmental pneumonia group and the bronchopneumonia group according to the changes of chest radiograph.Meanwhile,WBC,CRP and MP-IgG titers were measured in both groups.Results: Of the 219 children with MPP,65 cases( 29.6%) were in the segmental pneumonia group with an average age of( 93.98±14.57) months,and 154 cases( 70.4%) were in the bronchopneumonia group with an average age of( 54.73±16.43) months( P<0.05).The mean fever time was( 3.52±1.48) d in the segmental pneumonia group,and( 2.51±0.99) d in the bronchopneumonia group( P<0.05),the number of cases in the segmental pneumonia group with serum MP-IgG≥1 ∶ 640 was larger than that in the bronchopneumonia group( P<0.05).The serum CRP value of the segmental pneumonia group was higher than that of the bronchial pneumonia group( P<0.05).Conclusion: Pulmonary inflammation after MP infection was manifested as segmental pneumonia in children with older age,longer fever time,higher CRP levels and MP-IgG titer.It is suggested that the systemic inflammatory response is strong in children,and elderly children are prone to severe lung disease after MP infection.
