摘  要:目的分析成年女性血清羧化不全骨钙素(ucOC)、N端中段骨钙素(N-MID)、骨钙素未羧化率与骨密度等的关系,探讨骨钙素、羧化不全骨钙素和骨钙素羧化程度在成年女性骨量评价中的意义以及在骨质疏松诊断和治疗中的临床应用价值。方法选择2017年10月~2018年4月在潍坊市人民医院查体中心接受体检的149名成年女性为研究对象,分为青年骨量正常组、绝经前骨量正常组和绝经后组,绝经后组分为骨量正常组、骨量低下组、骨质疏松组3个亚组,采集空腹静脉血测定羧化不全骨钙素、N端中段骨钙素水平及其他相关生化指标,计算骨钙素未羧化率,应用SPSS24.0统计学软件分析上述数据。结果①绝经后低骨量组ucOC水平明显高于绝经后骨量正常组和绝经前骨量正常组(P<0.05)。②青年骨量正常女性组ucOC水平与N-MID、身高呈正相关;未羧化率水平与身高呈正相关。绝经后女性ucOC水平与腰椎L1-L4BMD呈负相关,与N-MID水平呈正相关。③ucOC水平确定绝经后女性骨量丢失的ROC曲线下面积为0.657,N-MID水平的ROC曲线下面积为0.763。当ucOC水平≥1.5349μg/L或N-MID水平≥12.8077μg/L时,绝经后女性骨量丢失加速,骨质疏松发生的风险增加。结论绝经后女性血清ucOC水平越高,腰椎骨密度越低,血清ucOC、N-MID、骨钙素未羧化率的变化与女性骨骼生长和绝经后女性骨量的减少、骨质疏松症的发生与发展存在相关关系。Objective To investigate the relationship between serum undercarboxylated osteocalcin,N-terminal osteocalcin levels,the uncarboxylated rate of osteocalcin with bone mineral density and other data in adult women,to explore their significance of assessing the bone mass of adult women and the clinical value of diagnosis and treatment of osteoporosis.Methods A total of 149 adult women receiving physical examination in physical examination center of the Weifang people’s hospital from October 2017 to April 2018 were selected.According to the results of the bone density examination,age,whether is menopause or not,the group was divided into the young adults with normal bone mass group,the premenopausal group with normal bone mass,the postmenopausal group.The last group was divided into three sub-groups with normal bone mass group,low bone mass group and osteoporosis group.Age,height,body weight,body mass index(BMI),body fat rate,BMD values of lumbar vertebra L1-L4 were detected,and peripheral blood samples were taken to test serum ucOC,N-MID and other biochemical indexes.The uncarboxylated rate of osteocalcin was calculated and statistical analyses of these indicators were proceeded by SPSS24.0.Results ①The ucOC level of the postmenopausal group with low bone mass was significantly higher than that of the postmenopausal group with normal bone mass(P<0.05) and the premenopausal group with normal bone mass(P<0.05).②In the young women with normal bone mass group,serum ucOC level was positively correlated with serum N-MID level and height,uncarboxylated rate was positively correlated with height.The ucOC level of the postmenopausal group was negatively correlated with lumbar L1-L4 BMD and positively correlated with serum N-MID level.③The area under the ROC curve at the ucOC level was 0.657 and the area under the ROC curve at the N-MID level was 0.763.When ucOC level was greater than or equal to 1.5349μg/L or N-MID level was greater than or equal to 12.8077μg/L,it indicateed that postmenopausal women’s bone loss a
