摘 要:目的探讨过敏性哮喘患儿血清白细胞介素-25(IL-25)、白细胞介素-17受体B(IL-17RB)水平及临床意义.方法选取2016年10月-2017年10月台州市第一人民医院收治的57例过敏性哮喘患儿为研究对象,将患儿分为轻度哮喘组(17例)、中度哮喘组(19例)、重度哮喘组(21例).选取同期肺功能正常的20例儿童为对照组.应用肺功能仪检测肺通气功能指标;应用血细胞分析仪检测外周血中性粒细胞数、白细胞总数及嗜酸性粒细胞数;采用ELISA检测外周血血清IL-25及IL-17RB水平.结果对照组FEV1、FVC、FEV1/FVC、PEF水平显著高于其余3组(均P<0.05).随哮喘病情加重,FEV1、FVC、FEV1/FVC、PEF水平明显下降(均P<0.05).4组外周血中性粒细胞数及白细胞总数比较差异均无统计学意义(均P>0.05);对照组嗜酸性粒细胞数显著低于其余3组(P<0.05).随哮喘病情加重,嗜酸性粒细胞数明显升高(P<0.05).对照组血清IL-25及IL-17RB水平显著低于其余3组(均P<0.05).随哮喘病情加重,血清IL-25及IL-17RB水平明显上升(P<0.05).哮喘患儿血清IL-25、IL-17RB水平均与FEV1、FVC、FEV1/FVC、PEF呈明显负相关(P<0.05),与嗜酸性粒细胞数呈明显正相关(P<0.05).哮喘患儿血清IL-25水平与IL-17RB水平呈明显正相关(P<0.05).结论IL-25和IL-17RB参与过敏性哮喘的发生及发展,推测二者有望成为检测过敏性哮喘疾病严重程度的重要参考指标.Objective To explore the levels of serum interleukin-25( IL-25) and interleukin-17 receptor B( IL-17 RB) in children with asthma and the clinical significance. Methods Fifty-seven children with asthma treated in the First People’s Hospital of Taizhou from October 2016 to October 2017 were selected and divided into mild asthma group( 17 children),moderate asthma group( 19 children),and severe asthma group( 21 children). Twenty children with normal pulmonary function during the same period were selected as control group.Spirometer was used to detect pulmonary ventilation function indexes. Hematology analyzer was used to detect neutrophil count,WBC count,and eosinophil count in peripheral blood. ELISA was used to detect the levels of IL-25 and serum IL-17 RB in peripheral blood.Results The levels of FEV1,FVC,FEV1/FVC,and PEF in control group were statistically significantly higher than those in the other three groups( P< 0. 05). With aggravation of asthma,the levels of FEV1,FVC,FEV1/FVC,and PEF decreased significantly( P< 0. 05).There was no statistically significant difference in neutrophil count and WBC count in peripheral blood among the four groups( P>0. 05).Eosinophil count in control group was statistically significantly lower than those in the other three groups( P< 0. 05). With aggravation of asthma,eosinophil count increased significantly( P<0. 05). The levels of serum IL-25 and IL-17 RB in control group were statistically significantly lower than those in the other three groups( P<0. 05). With aggravation of asthma,the levels of serum IL-25 and IL-17 RB increased significantly( P<0. 05). The levels of serum IL-25 and IL-17 RB in children with asthma were significantly negatively correlated with FEV1,FVC,FEV1/FVC,and PEF( P<0. 05),which were significantly positively correlated with eosinophil count( P<0. 05).
