支撑喉镜和纤维喉镜下声带息肉切除术临床疗效对照分析

摘　　要：目的对比分析支撑喉镜、纤维喉镜下声带息肉切除术治疗声带息肉的临床疗效。方法方便选取该院2015年12月-2017年12月收治的108例声带息肉患者为研究对象,随机分为A组和B组,每组54例,对A组患者实施支撑喉镜下声带息肉切除术治疗,对B组患者实施纤维喉镜下声带息肉切除术治疗,比较两组患者手术耗时、术中出血量、术后并发症发生情况及整体治疗效果。结果 A组手术耗时(21.9±4.6)min较B组(29.3±4.3)min短,两组比较差异有统计学意义(χ^2=8.226,P<0.05)。且A组患者术中出血量(6.5±1.1)mL较B组(13.5±1.9)m L少,两组比较差异有统计学意义(t=22.319,P<0.05);术后A组患者出现1例口腔黏膜水肿,并发症发生率为1.9%,B组患者在术后出现3例声带损伤,4例舌麻木,并发症发生率为13.0%,A组并发症发生率较B组低,差异有统计学意义(χ^2=4.860,P<0.05);A组整体治疗效果中66.7%治愈,31.4%明显改善,1.9%无效,治疗总有效率为98.1%。B组整体治疗效果中53.7%治愈,35.2%明显改善,11.1%无效,治疗总有效率为88.9%,A组疾病治疗总有效率较B组高,差异有统计学意义(χ^2=4.209,P<0.05)。结论支撑喉镜下声带息肉切除术治疗声带息肉的安全性和疗效均优于纤维喉镜下声带息肉切除术。Objective To compare and analyze the clinical efficacy of vocal cord polypectomy under the support laryngoscope and fiberoptic laryngoscope in the treatment of vocal cord polyps.Methods 108 patients with vocal cord polyps admitted to our hospital from December 2015 to December 2017 were convenient randomly divided into group A and group B,54 patients in each group.Group A of patients underwent vocal cord polypectomy under laryngoscope.For the treatment of group B,vocal cord polypectomy under fiberoptic laryngoscopy was performed.The time,intraoperative blood loss,postoperative complications and overall treatment effect of the two groups were compared.Results The time of operation in group A was (21.9±4.6) min shorter than that in group B (29.3±4.3) min.The difference between the two groups was statistically significant (χ^2=8.226,P<0.05).The intraoperative blood loss (6.5±1.1) mL in group A was less than that in group B (13.5±1.9) mL.The difference between the two groups was statistically significant (t=22.319,P<0.05).Oral mucosal edema,the incidence of complications was 1.9%.In group B,there were 3 cases of vocal cord injury and 4 cases of tongue numbness.The complication rate was 13.0%.The incidence of complication in group A was lower than that in group B.Statistically significant (χ^2=4.860,P<0.05);66.7% of the overall treatment effect of group A was cured,31.4% was significantly improved,1.9% was ineffective,and the total effective rate was 98.1%.In group B,53.7% of the overall treatment effect was cured,35.2% was significantly improved,11.1% was ineffective,and the total effective rate was 88.9%.The total effective rate of group A disease treatment was higher than that of group B.The difference was statistically significant (χ^2=4.209,P<0.05).Conclusion The safety and efficacy of vocal cord polypectomy under laryngoscopic laryngoscope for vocal cord polyps is better than that of vocal cord polypectomy under fiberoptic laryngoscopy.
