基层医院Foleys双导尿管球囊促宫颈成熟用于足月妊娠引产的效果研究

摘　　要：目的探究Foleys双导尿管球囊促宫颈成熟用于足月妊娠引产中的应用效果。方法选择2017年10月-2018年10月间该院接收的足月待产孕妇作为研究对象,从中随机抽选出病例数80名,将其按照入院时间顺序分为对照组、观察组,一组40名。其中对照组孕妇给予小剂量催产素促宫颈成熟引产,而观察组患者在对照组基础上,采取Foleys双导尿管自制球囊促宫颈成熟引产。对比两组孕妇引产前后Bishop评分、分娩方式、母婴结局等。结果观察组孕妇引产后Bishop评分为:(8.89±1.58)分,高于对照组孕妇(6.70±1.43)分,组间数据对比(t=6.4995 P<0.05)。观察组孕妇成功分娩率为95.00%,高于对照组孕妇的80.00%,组间数据对比(χ^2=4.1143 P<0.05)。对照组母婴产后并发症发生率分别为:42.50%、20.00%;高于观察组:15.00%、5.00%,差异有统计学意义(P<0.05)。两组新生儿Apgar评分分别为:(8.29±1.02)分(对照组)、(9.98±1.09)分(观察组),组间数据对比(t=7.159 9,P<0.05)。结论在妊娠引产中,使用Foleys双导尿管球囊能够有效促进宫颈成熟,缩短产程,有利于改善母婴结局,并提高新生儿Apgar评分,应用价值十分明显,值得推荐。Objective To investigate the application effect of Foleys double catheter balloon for cervical ripening in induction of labor in term pregnancy.Methods The full-term pregnant women who were admitted to our hospital from October 2017 to October 2018 were selected as the study subjects.80 cases were randomly selected from the hospital,and they were divided into the control group and the observation group according to the order of admission,40 cases each.In the control group,a small dose of oxytocin was used to induce cervical ripening,and in the observation group,the Foleys double catheter-made self-made balloon was used to induce cervical ripening.The Bishop scores,delivery methods,and maternal and child outcomes were compared before and after induction of labor in the two groups.Results The Bishop score of the pregnant women in the observation group was (8.89±1.58)points,which was higher than that of the control group (6.70±1.43) points.The data between the groups was compared(t=6.499 5,P<0.05).The successful delivery rate of pregnant women in the observation group was 95.00%,which was higher than that of the control group (80.00%).The data comparison between the groups(χ^2=4.114 3,P<0.05).The incidence rates of maternal and postnatal complications in the control group were 42.50% and 20.00%,respectively;higher than the observation group: 15.00%,5.00%,and the difference was statistically significant(P<0.05).The Apgar scores of the two groups were (8.29±1.02)points(control group) and (9.98±1.09)points (observation group).The data were compared between groups(t=7.1599,P<0.05).Conclusion Foleys double catheter balloon can effectively promote cervical ripening,shorten labor,improve maternal and infant outcomes,and improve neonatal Apgar score.
