牛黄上清胶囊联合葡萄糖酸氯己定含漱液治疗口腔溃疡的临床研究

摘　　要：目的探讨牛黄上清胶囊联合葡萄糖酸氯己定含漱液治疗口腔溃疡的临床疗效。方法收集2017年8月-2018年8月在南阳市中心医院治疗的口腔溃疡患者92例,根据就诊号分为对照组(46例)和治疗组(46例)。对照组饭后含漱葡萄糖酸氯己定含漱液,10 mL/次,每次含2~5 min,2次/d;治疗组在对照组基础上口服牛黄上清胶囊,0.9 g/次,3次/d。两组均治疗1周。观察两组患者临床疗效,同时比较治疗前后两组患者症状改善时间、血清炎症因子水平和SF-36量表评分。结果治疗后,对照组和治疗组临床有效率分别为82.61%和97.83%,两组比较差异具有统计学意义(P<0.05)。治疗后,治疗组患者疼痛缓解时间、溃疡面愈合时间均明显短于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,两组血清白细胞介素-1(IL-1)、IL-6、IL-8、肿瘤坏死因子-α(TNF-α)水平均显著降低(P<0.05),且治疗组上述血清炎症因水平子明显低于对照组(P<0.05)。治疗后,两组SF-36量表评分明显升高(P<0.05),且治疗组SF-36量表评分明显高于对照组(P<0.05)。结论牛黄上清胶囊联合葡萄糖酸氯己定含漱液治疗口腔溃疡可有效改善患者临床症状,降低机体炎症反应,提高患者生活质量。Objective To investigate the clinical efficacy of Niuhuang Shangqing Capsules combined with Chlorhexidine Gluconate Gargle in treatment of oral ulcer. Methods Patients(92 cases) with oral ulcer in Nanyang City Central Hospital from August 2017 to August 2018 were divided into control(46 cases) and treatment(46 cases) groups according to visiting number. Patients in the control group were gargling after meals administered with Chlorhexidine Gluconate Gargle, 10 mL/time, continued for 2 to 5 min, twice daily. Patients in the treatment group were po administered with Niuhuang Shangqing Capsules on the basis of the control group, 0.9 g/time, three times daily. Patients in two groups were treated for 1 week. After treatment, the clinical efficacy was evaluated, and the symptoms improvement time, inflammatory markers levels, and SF-36 scale scores in two groups before and after treatment were compared. Results After treatment, the clinical efficacy and in the control and treatment groups was 82.61% and 97.83% respectively, and there were differences between two groups(P < 0.05). After treatment, the remission time of pain and the healing time of ulcer in the treatment group was significantly earlier than that in the control group, with significant difference between two groups(P < 0.05). After treatment, the serum IL-1, IL-6, IL-8, and TNF-α levels in two groups were significantly decreased(P < 0.05), and which in the treatment group were significantly lower than those in the control group(P < 0.05). After treatment, the SF-36 scale scores in two groups were significantly increased(P < 0.05), and which in the treatment group were significantly higher than that in the control group(P < 0.05). Conclusion Niuhuang Shangqing Capsules combined with Chlorhexidine Gluconate Gargle in treatment of oral ulcer can effectively improve the clinical symptoms, reduce inflammation reaction, improve the quality of life.
