摘　　要：目的探讨不同氧疗时间对合并营养不良的重度慢性阻塞性肺疾病(COPD)的影响。方法将2016年1月至2017年1月安阳市人民医院收治的116例患者分为A组(6~8h/d,39例)、B组(15h/d,37例)和C组(24h/d,38例),观察3组患者1年后营养评定(MNA评分)、血清清蛋白等指标,评价不同氧疗时间对患者的影响。结果A组患者治疗前后MNA评分比较,差异无统计学意义(P>0.05),B组、C组患者治疗后MNA评分均明显改善,差异均有统计学意义(P<0.05),但组间治疗后MNA评分比较,差异无统计学意义(P>0.05);A组患者治疗后血清清蛋白明显下降,差异有统计学意义(P<0.05);B组、C组患者治疗后血清清蛋白比较,差异均无统计学意义(P>0.05)。结论15h/d、24h/d氧疗均可改善合并营养不良的重度COPD患者的营养状况。Objective To investigate the effects of time durations of oxygen therapy in the patients with severe chronic obstructive pulmonary disease on malnutrition status. Methods A total of 116 patients were included and grouped into A、B and C according to the duration of oxygen therapy(6-8 h daily,3 9;15 h daily,37;24 h daily,40),who had been diagnosed with COPD on malnutrition status from January 2016 to January 2017.Mini nutrition assessment(MNA score),Serum albumin were evaluated during a one-year intervention to investigate the effects in the patients. Results After one year,the group A showed no statistically significant difference in MNA( P =0.296),the serum albumin showed a reduction( P =0.02);In group B and C,there was an increase in MNA(10.00 vs. 11.00,11.25 vs. 13.00, P <0.05),and there was no statistically significant difference between them( P =0.150);the serum albumin showed no statistically significant difference in both group B and group C( P >0.05). Conclusion Oxygen therapy at 15h daily and 24h daily can improve the nutritional status of patients with COPD on malnutrition status;continuous oxygen therapy is recommended.
