浮针配合舒筋活络汤治疗肩关节周围炎的疗效观察

摘　　要：目的观察浮针配合口服舒筋活络汤治疗肩关节周围炎的临床疗效及其对肩关节功能、血清一氧化氮合酶(NOS)、中性粒细胞巨噬细胞集落刺激因子(GM-CSF)及三角肌表面肌电值的影响。方法将80例肩关节周围炎患者随机分为治疗组和对照组,每组40例。对照组采用口服舒筋活络汤治疗,治疗组在对照组基础上采用浮针治疗。观察两组治疗前后肩关节功能各项评分、血清NOS和GM-CSF水平及各穴三角肌表面肌电值的变化情况,并比较两组临床疗效。结果治疗组总有效率为95.0%,对照组为75.0%,两组比较差异具有统计学意义(P<0.05)。两组治疗后肩关节功能各项评分、血清NOS和GM-CSF水平、各穴三角肌表面肌电值与同组治疗前比较,差异均具有统计学意义(P<0.05)。治疗组治疗后肩关节功能各项评分、血清NOS和GM-CSF水平、各穴三角肌表面肌电值与对照组比较,差异均具有统计学意义(P<0.05)。结论浮针配合口服舒筋活络汤是一种治疗肩关节周围炎的有效方法,可显著改善患者肩关节功能,降低NOS和GM-CSF水平。Objective To observe the clinical efficacy of superficial needling plus Shu Jin Huo Luo decoction in treating periarthritis of shoulder,and its effect of shoulder joint function,serum nitric oxide synthase(NOS),granulocyte macrophage colony stimulating factor(GM-CSF),and the surface electromyography(sEMG)of deltoid.Method Eighty patients with periarthritis of shoulder were randomized into a treatment group and a control group,with 40 cases in each group.The control group was intervened by oral administration of Shu Jin Huo Luo decoction,and the treatment group was additionally given superficial needling.Before and after treatment,the functional scores of shoulder joint,the levels of serum NOS and GM-CSF and the deltoid sEMG of each acupoint in the two groups were observed,and the clinical efficacies of the two groups were compared.Result The total effective rate was 95.0%in the treatment group,versus 75.0%in the control group,and the between-group difference was statistically significant(P<0.05).After treatment,the functional scores of shoulder joint,the levels of serum NOS and GM-CSF and the deltoid sEMG of each acupoint all changed significantly in both groups(P<0.05).The functional scores of shoulder joint,the levels of serum NOS and GM-CSF and the deltoid sEMG of each acupoint in the treatment group were significantly different from those in the control group after treatment(P<0.05).Conclusion Superficial needling plus Shu Jin Huo Luo decoction is an effective method for periarthritis of shoulder.It can markedly improve the shoulder joint function and down-regulate the levels of NOS and GM-CSF.
