摘　　要：目的:通过检测糖尿病母亲婴儿(IDMs)血浆N氨基末端脑钠肽前体(Nt-proBNP)水平,探讨其在IDMs中的临床意义,同时对IDMs心脏LVEDDI、LVEF及Tei指数进行监测,探讨其临床意义,为早期积极治疗、降低新生儿近远期伤残率和病死率提供依据。方法:将100例IDMs患儿根据母孕期血糖控制情况分为IDMs1组(血糖控制不理想组)58例和IDMs2组(血糖控制良好组)42例,同时选取同一时期产科健康产妇娩出的100例婴儿作为对照组,比较3组心功能指标NT-proBNP、CK-MB与心脏LVEDDI、LVEF及Tei指数,同时对NT-proBNP浓度和心功能做相关分析。结果:IDMs1组心功能指标NT-proBNP、CK-MB与心脏LVEDDI、LVEF及Tei指数均较IDMs2组高,差异有统计学意义(P<0.05);IDMs1组心功能指标NT-proBNP、CK-MB与心脏LVEDDI、LVEF及Tei指数均较对照组高,差异有统计学意义(P<0.05);IDMs2组NT-proBNP较对照组高,差异有统计学意义,但IDMs2组CK-MB及心脏LVEDDI、LVEF及Tei指数与对照组比较差异无统计学意义。结论:对IDMs及早进行NT-proBNP检查,尽早干预,以减轻远期并发症,提高患儿生活质量。Objective: To detect the infants of diabetic mothers (IDMs) level of Nt-proBNP,to explore its clinical significance in IDMs,and monitor IDMs LVEDDI,LVEF and Tei index,explore their clinical significance for early treatment,and provide the basis for reducing neonatal short and long-term disability and mortality rates. Methods: 100 cases of IDMs patients according to maternal blood glucose control were divided into group IDMs 1 (poor glycemic control group) 58 cases and group IDMs 2 (good blood sugar control group) 42 cases,and 100 cases of infants during the same period in our hospital obstetric delivery of healthy pregnant women as control group,comparing heart function index,CK-MB and NT-proBNP,LVEDDI,LVEF and Tei index between the three groups,and the correlation analysis was done between NT-proBNP concentration and cardiac function. Results: IDMs 1 group's heart function index of NT-proBNP,CK-MB and heart LVEDDI,LVEF and Tei index were higher than IDMs 2 group,the difference was statistically significant ( P < 0.05 );IDMs 1 group's heart function index of NT-proBNP,CK-MB and heart LVEDDI,LVEF and Tei index were higher than the control group,the difference was statistically significant ( P <0.05);IDMs 2 group's NT-proBNP was higher than the control group,the difference was statistically significant,but IDMs2 group's CK-MB,LVEDDI,LVEF and Tei index were no statistically significant difference compared with the control group Conclusions: Early NT-proBNP examination of IDMs and early intervention to reduce the long-term complications and improve the quality of life in the children.
