摘 要:目的观察针刀松解对坐骨神经痛大鼠中枢各区域多巴胺(DA)、5-羟色安(5-HT)、去甲肾上腺素(NE)、八肽胆囊收缩素(CCK-8)、β-内啡肽(β-EP)、亮氨酸-脑腓呔(L-ENK)镇痛物质影响。方法将90 只清洁级SD 大鼠随机分作对照组、神经损伤组及针刀松解组;对照组:仅手术,不结扎坐骨神经;神经损伤组:结扎坐骨神经,无其他干预;针刀松解组:结扎坐骨神经,在造模成功后14、21、28 d 给予针刀松解干预。3 组在35 d 取中枢各区域组织,用放免检测法检测CCK-8 水平;ELISA 法检测DA、5-HT、NE 水平;免疫组化法检测β-EP、L-ENK 水平;及Western blotting 法检测L-ENK 水平。结果各区域针刀松解组较神经损伤组和对照组CCK-8、DA、NE、5-HT 水平升高(P<0.01);海马区域针刀松解组β-EP、L-ENK 水平升高(P<0.01)。结论针刀松解干预坐骨神经痛大鼠能促进中枢各区域DA、NE、5-HT、β-EP、L-ENK 等镇痛物质表达,起到内源性中枢镇痛的作用。Objective To observe the effects of acupotomy release on dopamine (DA), 5 hydroxysetron (5-HT),noradrenaline(NE),P substance(SP),octapeptide cholecystokinin (CCK-8),β-endorphin (β-EP),leucine- enkephalin (L-ENK) analgesic substances in different central regions. Methods Ninety clean SD rats were randomly divided into the control group, nerve injury group and acupotomy release group. For the control group only surgery was performed, no sciatic nerve ligation;For the nerve injury group,the sciatic nerve was ligated without other intervention. For the acupotomology group,the sciatic nerve was ligated, and acupotomology intervention was given on 14, 21 and 28 days after the successful modeling. All tissues in the central region were taken from the three groups at 35 days, and CCK-8 levels were detected by radioimmunoassay.The levels of DA, 5-HT and NE were detected by ELISA.β-EP and L-ENK levels were determined by immunohistochemistry. L-ENk levels were detected by Western blotting. Results The levels of CCK-8, DA, NE and 5-HT in the acupotomology group were higher than those in the nerve injury group and the control group (P< 0.01). The levels of β-EP and L-ENK were increased in the hippocampal acupotomy group (P< 0.01). Conclusion Acupotomy interven-tion in sciatica rats can promote the expressing of DA, NE, 5-HT,β-EP,L-ENK and other analgesic substances in central regions, and play an endogenous role in central analgesia.
