Effects of Qishen Granule(芪参颗粒) on Myocardial Fibrosis in Rats with Heart Failure after Myocardial Infarction

摘  要:目的探讨芪参颗粒治疗心肌梗死后心力衰竭大鼠心肌纤维化的可能作用机制。方法 50只SD大鼠随机分为假手术组15只、模型组12只、芪参颗粒组12只、非诺贝特组11只。除假手术组外,其余各组大鼠采用左心冠状动脉结扎诱导心肌梗死后心力衰竭大鼠模型。芪参颗粒组给予芪参颗粒18.66 g/(kg·d)灌胃,非诺贝特组给予非诺贝特胶囊10 mg/(kg·d)灌胃,假手术组和模型组给予同体积生理盐水灌胃,连续给药28天。检测各组大鼠心功能,分别观察各组大鼠心肌组织病理变化,并采用Western blot法检测大鼠心肌组织中转化生长因子β(TGF-β)、Smad3、Wnt1a、β-catenin、Wnt1诱导的分泌蛋白1 (WISP-1)、Wnt5a蛋白含量。结果与假手术组比较,模型组左室射血分数(EF)、短轴缩短率(FS)均显著降低,左室舒张末内径(LVIDd)、左室收缩末内径(LVIDs)均显著增高(P<0.01);与模型组比较,芪参颗粒组EF、FS显著升高,LVIDd、LVIDs显著降低,非诺贝特组EF、FS显著升高,LVIDs显著降低(P<0.05或P<0.01)。HE染色显示,模型组大鼠心肌细胞排列紊乱,心肌间质出现大量炎性细胞浸润,胶原过度沉积,纤维明显增生,丧失正常结构;芪参颗粒组与假手术组细胞排列整齐,细胞形态基本无变化,仅表现少量胶原沉积,心肌纤维化得以改善。与假手术组比较,模型组TGF-β、Smad3、Wnt1a、β-catenin、WISP-1、Wnt5a蛋白表达增高(P<0.05或P<0.01);与模型组比较,芪参颗粒组和非诺贝特组TGF-β、Smad3、Wnt1a、β-catenin、WISP-1、Wnt5a蛋白降低(P<0.05或P<0.01),芪参颗粒组与非诺贝特组各指标比较差异均无统计学意义(P> 0.05)。结论芪参颗粒能够显著改善心肌梗死后心力衰竭大鼠心肌纤维化,且与非诺贝特胶囊效果相当,其机制可能与抑制心肌组织TGF-β与Wnt通路有关。Objective To explore the possible mechanism of Qishen Granule(芪参颗粒) in the treatment of myocardial fibrosis in rats with heart failure after myocardial infarction.Methods A total of 50 SD rats were randomly divided into a sham operation group with 15 rats,a model group with 12 rats,a Qishen Granule group with 12 rats and a fenofibrate group with 11 rats.Except for the sham operation group,the rats in the other groups performed left heart coronary artery ligation to induce heart failure model after myocardial infarction.The Qishen Granule group was given18.66 g/(kg·d) Qishen Granule by gavage.The fenofibrate group was given fenofibrate 10 mg/(kg·d) by gavage.The sham operation group and the model group were given the same volume of normal saline.The rats were administered for 28 days.The cardiac function and the pathological changes of myocardial tissue in each group were observed.The expression of transforming growth factor β(TGF-β),Smad3,Wnt1 a,β-catenin,Wnt1 induced secreted protein 1(WISP-1),and Wnt5 a protein content in rat myocardium were detected by Western blot.Results Compared with the sham operation group,the left ventricular ejection fraction(EF) and fraction shortening(FS) of the model group were significantly decreased,and the left ventricular internal diameter at end-diastole(LVIDd) and the left ventricular internal diameter at end-systolic(LVIDs) were significantly increased(P<0.01);Compared with the model group,the EF and FS of the Qishen Granule group were significantly increased,and the LVIDd and LVIDs were significantly decreased.The EF and FS of the fenofibrate group were significantly increased,and the LVIDs were significantly decreased(P<0.05 or P<0.01).HE staining showed that the myocardial cells in the model group were disordered,and a large number of inflammatory cells infiltrated in the myocardial interstitial,excessive collagen deposition,significant fibrosis and loss of normal structure.The cells of the Qishen Granule group and the sham operation group were arranged neatly,
