摘  要:目的探讨冠心病(CHD)患者心率变异性的线性分析方法中频域及时域指标与非线性分析方法Lorenz散点图指标的相关性,寻找更准确的HRV分析方法。方法收集100例冠心病患者作为冠心病组,并以100例健康体检者作为对照组。监测、比较两组24h动态心电图Lorenz散点图的长度L、宽度W、面积A、散点图形态与时域指标(SDNN、r MSSD及p50)及频域指标(HF及LF),并将两组散点图指标与频域、时域指标行相关性分析。结果与对照组相比,冠心病组的Lorenz散点图的L、W、A与SDNN、LF均显著下降,冠心病组Lorenz散点图多呈彗星状、鱼雷状、短棒状、梭状,而对照组则以彗星状为主,Lorenz散点图的长度L与时域指标SDNN、r MSSD及p50呈显著相关,散点图宽度W与频域指标HF及LF呈显著相关。结论与线性HRV分析方法相比,Lorenz散点图能更加灵敏地反映HRV的变化,可作为临床分析HRV一种重要工具。Objective To explore the correlation between the frequency domain and time domain indicators and the nonlinear method Lorenz plot index in the linear method analysis method of heart rate variability(HRV) in patients with coronary heart disease(CHD), and to find a more accurate method for analyzing HRV. Methods One hundred patients with CHD were enrolled as CHD group, and 100 healthy subjects were used as control group.The time-domain and frequency-domain indicators of the 24-hour ambulatory electrocardiogram Lorenz plot in CHD group were compared with control group. Results Compared with the control group, SDNN, r MSSD, p50 and scatter plot L,W,A in CHD group were significantly decreased. The Lorenz plot of CHD group were mostly comet-shaped,torpedo-like, short-rod-shaped, and fusiform. The control group is mainly comet-like. The length L of the Lorenz scatter plot is significantly correlated with the time domain indicators SDNN, r MSSD and p50. The width of the scatter plot is significantly correlated with the frequency domain indicators HF and LF. Conclusions Compared with the linear method HRV analysis method, Lorenz plot can more sensitively reflect the change of HRV, which can be used as an important means for clinical analysis of HRV.
