椎体成形术与椎体后凸成形术治疗多发性骨质疏松脊柱骨折的临床价值分析

摘　　要：目的探讨椎体成形术(PVP)与椎体后凸成形术(PKP)治疗多发性骨质疏松脊柱骨折的临床价值。方法 68例多发性骨质疏松脊柱骨折患者,按照手术方法不同分为对照组32例和观察组36例,对照组采用PVP治疗,观察组采用PKP治疗,比较两组手术、椎体恢复和疼痛情况,临床疗效及并发症发生情况。结果对照组手术时间、治疗费用均少于观察组(P<0. 05);两组住院时间、术后ODI及临床总有效率比较,差异均无统计学意义(P>0. 05);两组术后1天、3月和6月疼痛评分均较术前降低(P<0. 05),但组间比较差异均无统计学意义(P>0. 05);观察组并发症发生率低于对照组(P<0. 05)。结论PVP与PKP治疗多发性骨质疏松性骨折患者的临床疗效类似,其中PVP所用时间和住院费用较少,PKP术后并发症较低,由此可见,为进一步改善患者预后,PKP手术方案更适用多发性骨质疏松骨折患者的治疗。Objective To investigate the clinical value of vertebroplasty (PVP) and kyphoplasty (PKP) in the treatment of multiple osteoporotic spinal fractures.Methods Sixty-eight patients with multiple osteoporotic spinal fractures were divided into control group (n = 32) and observation group (n = 36) according to the operation methods.The control group was treated with PVP besides basic treatment,and the observation group was treated with PKP besides basic treatment.The operation,vertebral recovery,pain and clinical effect as well as postoperative complications were compared between the two groups.Results The operation time was shorter and the cost was less of the control group than those of the observation group (P < 0. 05). However,there was no statistically significant difference in the length of stay postoperative ODI and total effective rate between the two groups (P>0. 05).The pain score after 1 day,3 and 6 months of operation was lower when compared to before operation (P < 0. 05) but there was no difference between the two groups (P>0. 05).The incidence rate of complications in the observation group was lower than that in the control group (P < 0. 05).Conclusion The clinical effect of PVP and PKP in the treatment of multiple osteoporotic fractures is similar.The time and cost of PVP patients are shorter or less,but the postoperative complications of PKP patients are significantly lower.Thus,in order to further improve the prognosis of patients,PKP surgery is more suitable for the treatment of patients with multiple osteoporotic fractures.
