摘　　要：目的分析合并非酒精性脂肪肝的2型糖尿病患者的血清脂联素水平与其颈动脉斑块特点的相关性。方法纳入临床诊断2型糖尿病合并非酒精性脂肪肝患者156例,检测血清脂联素浓度,行颈动脉超声检查评估颈动脉内膜-中膜厚度,检出颈动脉斑块并评定其性质。对比三组血脂、颈动脉内膜-中膜厚度及血清脂联素水平,并判断血清脂联素水平与颈动脉中膜-内膜厚度的相关性。结果不稳定斑块组血清脂联素水平高于无斑块组;不稳定斑块组血清non-HDL-c水平高于稳定斑块组及无斑块组,差异均有统计学意义(P<0. 05)。血清脂联素水平与颈动脉内膜-中膜厚度呈负相关(r=-0. 697,P=0. 035)。结论血清脂联素、非高密度脂蛋白胆固醇可能与合并非酒精性脂肪肝的2型糖尿病患者颈动脉斑块的形成及斑块性质有关。Objective To analyze the relationship between serum adiponectin level and characteristics of carotid ultrasonography in T2DM patients with non-alcoholic fatty liver disease(NAFLD).Methods A total of 156 T2DM patients with NAFLD were randomly selected for this study.The serum adiponectin was detected.The intima-media thickness of carotid,carotid plaque and its characteristics were examined with carotid artery ultrasonography.The patients were divided into 3 groups according to existence and characteristics of carotid plaque. The serum adiponectin,carotid intima-media thickness and blood lipid level were compared among the three groups.The relationship between the serum adiponectin and the carotid intima-media thicknesswas assessed.Results Serum adiponectin level in the unstable plaque group was higher than non-plaque group and non-HDL-c in the unstable plaque group was higher than both non-plaque group and stable plaque group. The differences were statistically significant. The serum adiponectin was negatively related with carotid intima-media thickness(r =-0. 697,P = 0. 035).Conclusion Adiponectin and non-HDL cholesterol are both associated with carotid plaque characteristics in T2DM patients with NAFLD.
