经皮椎间孔镜技术治疗退变性腰椎管狭窄症的临床研究

摘　　要：目的探讨经皮椎间孔镜技术治疗退变性腰椎管狭窄症的临床疗效。方法将90例退变性腰椎管狭窄症患者按随机数字表法分为对照组和观察组各45例,分别采用传统手术治疗和经皮椎间孔镜技术治疗,比较两组手术治疗指标(手术时间、术中出血量、术后下床时间、住院时间)、疼痛创伤指标(VAS评分)、运动恢复指标(Nakai评定)、安全性指标(神经损伤、硬膜撕裂、感染等并发症)。结果观察组手术时间、术中出血量、术后下床时间、住院时间均优于对照组(P<0. 05)。术后观察组VAS评分低于对照组(P<0. 05)。运动恢复总有效率和并发症发生率方面,观察组为95. 56%(43/45)、8. 89%(4/45),对照组为82. 22%(37/45)、24. 44%(11/45),差异有统计学意义(P<0. 05)。结论经皮椎间孔镜技术治疗退变行腰椎管狭窄效果明显,值得推广。Objective To explore the clinical effect of percutaneous transforaminal endoscopy in the treatment of degenerative lumbar spinal stenosis.Methods Ninety patients with degenerative lumbar spinal stenosis were randomly divided into control or observation group,45 in each group.They were treated with traditional surgery and percutaneous transforaminal endoscopy,respectively. The surgical treatment indexes such as operation time,intraoperative bleeding volume,time out of bed after operation and hospital stay,pain and trauma index(VAS score),motor recovery index( Nakai evaluation) and safety indicators such as nerve injury,dural tear,infection and other complications were compared between the two groups. Results The observation group was better than the control group in terms of operation time,intraoperative bleeding volume,time to get out of bed after operation and hospitalization time(P< 0. 05).After operation,the VAS score of the observation group was lower than that of the control group(P< 0. 05).The total effective rate and the incidence of complications were 95. 56%(43/45) and 8. 89%(4/45) in the observation group,respectively. The total effective rate and the incidence of complications were 82. 22%(37/45) and 24. 44%(11/45) in the control group,respectively. The differences were statistically significant(P< 0. 05).Conclusion Percutaneous transforaminal endoscopy is effective in the treatment of degenerative lumbar spinal stenosis.It is worth popularizing.
