经尿道前列腺等离子切除术治疗前列腺增生的疗效及影响因素分析

摘　　要：目的分析经尿道前列腺等离子切除术(PKRP)治疗前列腺增生(BPH)的疗效及影响因素。方法接受PKRP治疗的227例BPH患者,统计手术情况,评价疗效,分析PKRP疗效的影响因素。结果 227例患者均顺利完成手术,手术时间(52. 0±10. 66) min,切除前列腺组织质量(47. 12±8. 69) g,术中出血量(135. 77±20. 25) ml,前列腺包膜切穿率3. 08%(7/227),尿失禁发生率1. 32%(3/227),治疗有效率84. 14%(191/227);按疗效分组,有效组病程短于无效组,国际前列腺症状评分(IPSS)评分、残余尿量、术前尿路感染比重及术后导尿管留置时间低于无效组,前列腺体积、膀胱收缩指数(BCI)、膀胱出口梗阻指数(BOOI)高于无效组(P<0. 05)。IPSS、前列腺体积、残余尿量、BCI、BOOI、术前尿路感染为PKRP疗效的独立影响因素(P<0. 05)。结论 PKRP治疗BPH疗效佳,但术前应重视上述因素对手术疗效的影响,评估手术风险,最大限度降低IPSS、前列腺体积等影响因素对疗效的影响。Objective To analyze the curative effect of bipolar plasmakinetic resection of prostate(PKRP) in the treatment of benign prostatic hyperplasia(BPH) and its influencing factors.Methods The clinical data of 227 patients with BPH who underwent PKRP were retrospectively analyzed.The surgical conditions were statistically analyzed.The curative effect was evaluated.The influencing factors for the curative effect of PKRP were analyzed. Results All patients had successfully completed the operation. The operation time,mass of excised prostate tissue,intraoperative blood loss,cut-through rate of prostate capsule,incidence of urinary incontinence and response rate of treatment were(52. 0 ± 10. 66) min,(47. 12 ± 8. 69) g,(135. 77 ± 20. 25) ml,3. 08%(7/227),1. 32%(3/227) and84. 14%(191/227),respectively.They were divided into effective and ineffective groups according to the curative effect. The disease course in the effective group was shorter than that in the ineffective group.Compared to the ineffective group,the international prostate symptom score(IPSS),residual urine volume,proportion of preoperative urinary tract infection and postoperative catheter indwelling time were lower or shorter while the prostate volume,bladder contraction index(BCI) and bladder outlet obstruction index(BOOI)were higher(P <0. 05). We found that IPSS,prostate volume,residual urine volume,BCI,BOOI preoperative urinary tract infection were independent factors affecting the curative effect of PKRP(P<0. 05).Conclusion The curative effect of PKRP is relatively better in the treatment of BPH.The influences of the above factors on the surgical curative effect should be emphasized before operation to reduce the influence of IPSS and prostate volume on the curative effect of the operation.
