超声与CT诊断鼻骨骨折的应用价值比较

摘　　要：目的比较超声与CT诊断鼻骨骨折的临床价值。方法回顾性分析经手术确诊的186例鼻骨骨折患者的术前超声和CT检查资料,比较两种检查方法对鼻骨骨折的总体检出率及不同类型骨折的阳性检出率。结果超声和CT对鼻骨骨折的总体检出率分别为91.4%、94.6%,差异无统计学意义。超声对单纯线性骨折的阳性检出率(55.4%)高于CT(48.9%),差异有统计学意义(P<0.05);超声对粉碎性骨折的阳性检出率(29.6%)与CT(27.9%)相近,差异无统计学意义;超声对复合性骨折阳性检出率(6.5%)低于CT(17.7%),差异有统计学意义(P<0.05)。结论超声安全、价廉、便捷,对鼻骨骨折的检出率与CT相当,可作为鼻骨骨折影像学检查的一项重要补充,值得临床推广应用。Objective To compare the clinical value of ultrasound and CT scanning in the diagnosis of nasal bone fracture. Methods The preoperative ultrasound and CT scanning data of totally 186 nasal bone fracture patients confirmed by operation were retrospectively analyzed. The overall detection rates of nasal bone fractures and the positive rate of different types of fractures were compared between the two methods. Results The overall detection rates of nasal bone fracture by ultrasound and CT were 91.4% and 94.6%,respectively,and the difference was not statistically significant.The positive rate of ultrasound in detecting simple linear fracture(55.4%)was higher than that of CT(48.9%),the difference was statistically significant(P<0.05), the positive rate of ultrasound in detecting comminuted fracture(29.6%)was similar to that of CT(27.9%),and the difference was not statistically significant. The positive rate of ultrasound(6.5%)for complex fracture was lower than that of CT(17.7%), the difference was statistically significant(P<0.05). Conclusion Ultrasound is safe,inexpensive and convenient,which can be used as an important supplement to the imaging examination of nasal bone fracture and is worthy of clinical application.
