摘  要:目的探讨早期2型糖尿病患者血清补体C3、C4水平与肥胖、胰岛素抵抗的关系。方法选择178例早期2型糖尿病患者,依据体质指数分为正常体质指数组45例、超重组63例、肥胖组70例,同时纳入37例健康者作为对照组。比较4组的血清C3、C4水平;分析2型糖尿病患者补体水平与体质指数、血脂、稳态模型评估胰岛素抵抗(HOMA-IR)指数的相关性。结果对照组、正常体质指数组、超重组、肥胖组的血清C3水平依次升高(均P<0.05);正常体质指数组、超重组、肥胖组的血清C4水平均高于对照组(均P<0.05),但3组间差异无统计学意义(P>0.05)。在2型糖尿病患者中,血清C3水平与HOMA-IR指数、体质指数呈正相关(均P<0.05),血清C4水平与体质指数、HOMA-IR指数无相关性(均P>0.05);血清C3、C4水平与血脂水平均无相关性(均P>0.05)。结论补体C3可能在2型糖尿病合并肥胖发病机制中发挥重要作用,并影响胰岛素抵抗。Objective To investigate the relationship of serum complements C3/C4 level with obesity and insulin resistance in patients with early type 2 diabetes mellitus(T2DM). Methods A total of 178 early T2DM patients were selected and divided into normal body mass index group( n =45),overweight group( n =63) and obesity group ( n =70) according to their body mass index,and 37 healthy individuals were enrolled as control group.Serum C3 and C4 levels were compared among the four groups;the correlation of complements levels with body mass index,blood lipids,and homeostasis model assessment-insulin resistance(HOMA-IR) index was assessed in T2DM patients. Results Serum C3 level increased in the order of the control group,the normal body mass index group,the overweight group,and the obesity group(all P <0.05);the serum C4 level in the normal body mass index group,overweight group and obesity group was higher than that in the control group(all P <0.05),but no statistically significant difference was observed among these three groups( P >0.05).In T2DM patients,HOMA-IR index or body mass index positively correlated with serum C3 level(all P <0.05),but did not correlate with serum C4 level(all P >0.05);no correlation was observed between serum C3/C4 level and blood lipids level(all P >0.05). Conclusion Complement C3 may exert an important role in pathogenesis of T2DM complicated with obesity,and can affect insulin resistance.
