雷公藤多苷片联合泼尼松治疗肾病综合征的临床效果

摘　　要：目的观察雷公藤多苷片联合泼尼松治疗肾病综合征的近期效果。方法选取我院2016年1月~2017年1月收治的80例肾病综合征患者,随机分为对照组与观察组,每组各40例。对照组采用泼尼松治疗,观察组在对照组的基础上联合雷公藤多苷片进行治疗。比较两组治疗前后的血浆清蛋白、尿素氮、血肌酐、24h尿蛋白等水平变化以及近期效果、不良反应总发生率、复发率。结果两组治疗后的血浆清蛋白、尿素氮、血肌酐、24h尿蛋白等水平较治疗前均显著改善,且观察组治疗后的血浆清蛋白水平高于对照组,尿素氮、血肌酐、24h尿蛋白等水平均低于对照组,差异有统计学意义(P<0.05)。观察组的治疗总有效率为90.0%,高于对照组的75.0%,差异有统计学意义(P<0.05)。观察组的不良反应总发生率为7.5%,低于对照组的20.0%,差异有统计学意义(P<0.05)。随访12个月,观察组的复发率为10.0%,低于对照组的25.0%,差异有统计学意义(P<0.05)。结论雷公藤多苷片联合泼尼松治疗肾病综合征的近期效果非常显著,既可显著改善患者的临床指标和各种症状,又能降低不良反应发生率和复发率,安全性较高,值得临床应用推广。Objective To observe the short-term effect of Tripterygium Wilfordii Polyglycoside Tablets combined with Prednisone in the treatment of nephrotic syndrome. Methods From January 2016 to January 2017,80 patients with nephrotic syndrome were selected and randomly divided into the control group and the observation group,40 cases in each group. The control group was treated with Prednisone,and the observation group was treated with Tripterygium Wilfordii Polyglycosides Tablets on the basis of the control group. The level changes of plasma albumin,urea nitrogen,serum creatinine,24 hours urine protein before and after treatment,short-term effect,the total incidence rate of adverse reaction and recurrence rate were compared between the two groups. Results The levels of plasma albumin,urea nitrogen,creatinine and 24-hour urinary protein in the two groups after treatment significantly improved compared with those before treatment,the plasma albumin level in the observation group after treatment was higher than that in the control group,and the levels of urea nitrogen,creatinine and 24-hour urine protein in the observation group after treatment were lower than those in the control group,the differences were statistically significant (P<0.05). The total effective rate of the observation group was 90.0%,which was higher than 75.0% of the control group,and the difference was statistically significant (P<0.05). The total incidence rate of adverse reactions in the observation group was 7.5%,which was lower than 20.0% in the control group,and the difference was statistically significant (P<0.05). Following up for 12 months,the recurrence rate of the observation group was 10.0%,which was lower than 25.0% of the control group,the difference was statistically significant (P<0.05). Conclusion Tripterygium Wilfordii Polyglycoside Tablets combined with Prednisone is very effective in the treatment of nephrotic syndrome. It can significantly improve the clinical indexes and symptoms,and reduce the incidence rate of adverse reaction and
