摘　　要：目的探讨与分析Lauge-Hansen旋前-外旋型踝关节骨折伴下胫腓联合分离行下胫腓螺钉固定治疗的效果。方法选取广东省东莞市第八人民医院2016年1月~2018年1月收治的88例Lauge-Hansen旋前-外旋型踝关节骨折伴下胫腓联合分离患者,采取随机数字表法分为固定组与未固定组,每组各44例。固定组给予下胫腓螺钉固定治疗,未固定组未给予下胫腓螺钉固定治疗,比较两组的临床疗效、下胫腓间隙及下胫腓重叠距离以及术后感染、术后骨折对位不良及术后下肢不等长的发生率,随访1年的下肢功能评分。结果两组的临床总有效率比较,差异无统计学意义(P>0.05)。两组的术后感染、术后骨折对位不良及术后下肢不等长发生率比较,差异无统计学意义(P>0.05)。两组术前的下胫腓间隙、下胫腓重叠距离与术后6个月比较,差异无统计学意义(P>0.05)。两组术后6个月的下胫腓间隙、下胫腓重叠距离比较,差异无统计学意义(P>0.05)。两组术后随访6、12个月的下肢功能评分与术前比较,差异有统计学意义(P<0.05)。两组术后随访6、12个月的下肢功能评分比较,差异无统计学意义(P>0.05)。结论对于Lauge-Hansen旋前-外旋型踝关节骨折伴下胫腓联合分离患者而言,是否给予下胫腓螺钉固定治疗不会对下肢功能的恢复带来较大影响。Objective To investigate and analyze the effect of lower tibiofibular screw fixation in the treatment of Lauge-Hansen pronation-external rotation ankle fracture combined with lower tibiofibular combination separation. Methods A total of 88 patients with Lauge-Hansen pronation-external ankle fracture and lower tibiofibular combination separation of the Eighth People′s Hospital of Dongguan City in Guangdong Province from January 2016 to January 2018 were enrolled. They were divided into the fixed group and the unfixed group according to random number table method, 44 cases in each group. The fixed group was treated with sacral screw fixation, and the unfixed group was not treated with sacral screw fixation. The clinical efficacy, lower tibiofibular space and lower tibiofibular overlap distances, and the incidence of postoperative infection, postoperative fracture dysplasia and postoperative lower limb unequal length were compared between the two groups. The lower limb function score was followed up for 1 year. Results There was no significant difference in the total clinical effective rate between the two groups (P>0.05). There was no significant difference in the incidence of postoperative infection, postoperative fracture dysplasia and postoperative lower limb unequal length between the two groups (P>0.05). There were no significant differences in the lower tibiofibular space and lower tibiofibular overlap distances between before operation and 6 months after operation in the two groups (P>0.05). There was no significant difference in the lower tibiofibular space and lower tibiofibular overlap distances at 6 months after operation between the two groups (P>0.05). The lower extremity function scores of the two groups after 6 and 12 months follow-up were significantly different from those before operation (P<0.05). There was no significant difference in lower limb function score between the two groups after 6 and 12 months of follow-up (P>0.05).
