致康胶囊联合九华膏治疗环状混合痔术后患者的临床价值

摘　　要：目的探讨致康胶囊联合九华膏治疗环状混合痔术后患者的临床价值。方法按照随机数字表法将西安市中医医院2016年9月至2018年8月收治的72例环状混合痔患者分为观察组与对照组,各36例。两组均行混合痔外剥内扎术,且在术后常规抗感染及清洗基础上,对照组给予九华膏外涂,观察组给予九华膏外涂+致康胶囊口服。比较两组的临床疗效、创面愈合情况、时间、血小板源性生长因子(PDFG)、碱性成纤维生长因子(bFGF)水平及术后并发症发生情况。结果观察组的治疗总有效率显著高于对照组,差异有统计学意义(P<0.05);术后第10天,观察组创面疼痛、水肿、渗液、面积与肉芽组织评分均低于对照组,且愈合时间短于对照组,差异有统计学意义(P<0.05);术后第10天,观察组血清PDFG、bFGF水平高于对照组,差异有统计学意义(P<0.05);两组术后并发症总发生率无显著差异(P>0.05)。结论给予环状混合痔患者术后致康胶囊口服联合九华膏外涂,可取得显著效果,有利于促进创面早期愈合,其可能与上调血清PDFG、bFGF表达有关。Objective To investigate the clinical value of Zhikang capsule combined with Jiuhua plaster in the treatment of patients with circular mixed hemorrhoid after operation.Methods According to the random number table method,72 patients with circular mixed hemorrhoid admitted in Xi'an Traditional Chinese Medicine Hospital from September 2016 to August 2018 were divided into observation group and control group,with 36 cases in each group.Both groups were treated with external stripping and internal ligation of mixed hemorrhoid.On the basis of routine anti-infection and cleaning after operation,the control group was treated with external application of Jiuhua plaster,while the observation group was treated with external application of Jiuhua plaster and Zhikang capsule orally.The clinical efficacy,status and time of wound healing,levels of platelet-derived growth factor(PDFG)and basic fibroblast growth factor(bFGF)and postoperative complications were compared between the two groups.Results The total effective rate of treatment in the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).On the 10th day after the operation,the scores of wound pain,edema,exudation,area and granulation tissue in the observation group were lower than those in the control group,the healing time was shorter than that of the control group,and the differences were statistically significant(P<0.05).On the 10th day after the operation,the levels of serum PDFG and bFGF in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of postoperative complications between the two groups(P>0.05).Conclusions Zhikang capsule orally combined with external application of Jiuhua plaster on patients with circular mixed hemorrhoid after operation can achieve significant effects and promote early wound healing,which may be related to the up-regulated expression
