阳和健脾汤联合西医常规治疗下肢动脉硬化闭塞症疗效观察

摘　　要：目的观察阳和健脾汤联合西医常规治疗下肢动脉硬化闭塞症(ASO)的疗效。方法将100例下肢ASO患者按照随机数字表法分为2组。对照组50例予西医常规治疗;治疗组50例在对照组治疗基础上予阳和健脾汤治疗。2组疗程均4周。比较2组治疗前后主观症状(疼痛、皮肤温度感、皮肤色泽)评分、客观指标及体征[踝肱指数(ABI)、经皮氧分压(TcPO2)、足背皮温、间歇性跛行距离]、足背动脉血流动力学参数(血管内径、血流量、峰值血流速度、搏动指数、阻力指数),并统计中西医临床疗效。结果治疗后2组各主观症状评分均降低(P<0.05),治疗组均低于对照组(P<0.05)。治疗后2组ABI、TcPO2及足背皮温均升高(P<0.05),间歇性跛行距离均延长(P<0.05),治疗组均高于对照组(P<0.05)。治疗后2组足背动脉血管内径、血流量、峰值血流速度、搏动指数、阻力指数均改善(P<0.05),且治疗组改善均优于对照组(P<0.05)。治疗组中医疗效总有效率(94.34%)优于对照组(82.97%,P<0.05)。治疗组西医疗效总有效率(86.79%)优于对照组(78.72%,P<0.05)。结论阳和健脾汤联合西医常规治疗下肢ASO疗效确切,能改善患者临床症状、体征,加快患肢血流速度。Objective To observe the clinical efficacy of Yanghe Jianpi decoction combined with routine western medicine on lower extremity arteriosclerosis obliterans(ASO). Methods 100 patients with ASO of lower extremity were divided into two groups according to random number table method. 50 cases in control group were treated by routine western medicine. 50 cases in treatment group were treated by Yanghe Jianpi decoction on the basis of the control group. The course of treatment was 4 weeks in both groups. The subjective symptoms(pain, skin temperature, skin color) scores, objective indicators and signs [ ankle-brachial index(ABI), transcutaneous partial pressure of oxygen(TcPO2), dorsal skin temperature, intermittent claudication distance ], dorsal pedal artery hemodynamic parameters(intravascular diameter, blood flow, peak blood flow velocity, pulsating finger) were compared between the two groups before and after treatment. Statistical analysis was made on the clinical efficacy of integrated traditional Chinese and western medicine. Results After treatment, the subjective symptom scores of the two groups were decreased(P<0.05), and the treatment group was lower than the control group(P<0.05). After treatment, ABI, TcPO2 and dorsal skin temperature were increased in both groups(P<0.05), and the interval claudication distance was prolonged(P<0.05) and the treatment group was higher than the control group(P<0.05). After treatment, the internal diameter, blood flow, peak blood flow velocity, pulsation index and resistance index of dorsal pedal artery were improved in both groups(P<0.05), and the improvement in treatment group was better than that in control group(P<0.05). The total effective rate of the treatment group(94.34%) was better than that of the control group(82.97%, P<0.05). The total effective rate of western medicine in the treatment group(86.79%) was better than that in the control group(78.72%, P<0.05).
