早期温和灸对血液透析患者自体动静脉内瘘术后成熟的影响

摘　　要：目的观察早期温和灸对血液透析患者自体动静脉内瘘(AVF)术后成熟的影响。方法将60例行血液透析的自体AVF术后患者按照随机数字表法分为2组,每组30例。对照组予常规护理及宽谱远红外线照射治疗。观察组在对照组基础上加清艾条温和灸治疗。比较2组术后24 h及术后3个月头静脉血流量、内径,术后3 d自体AVF术切口疼痛程度,术后3个月内瘘成熟率及并发症情况。结果术后24 h,2组头静脉血流量、内径比较差异无统计学意义(P>0.05);术后3个月,2组头静脉血流量、内径均较本组术后24 h增加(P<0.05),且观察组高于对照组(P<0.05)。观察组术后3 d疼痛率(66.67%)低于对照组(93.33%,P<0.05)。观察组内瘘生理学成熟率(93.33%)及临床成熟率(86.67%)均高于对照组(83.33%,80.00%),但比较差异无统计学意义(P>0.05)。观察组并发症总发生率(10.00%)低于对照组(36.67%,P<0.05)。结论早期温和灸能改善血液透析患者自体AVF术后内瘘功能,提高内瘘成熟率,减轻术口疼痛,临床并发症少。Objective To observe the effects of early mild moxibustion on postoperative maturation of autologous arteriovenous fistula(AVF) in hemodialysis patients. Methods 60 patients with autologous AVF after hemodialysis were divided into two groups according to the random number table method, with 30 cases in each group. The control group was treated by routine nursing and broad-spectrum far-infrared radiation. The observation group was treated by moxa stick mild moxibustion on the basis of the control group. The blood flow and internal diameter of the cephalic venous at 24 h and 3 months after the operation, the pain degree of autologous AVF incision at 3 days after the operation, the fistula maturity rate and complications at 3 months after the operation were compared between the two groups. Results There was no statistically significant difference in cephalic vein blood flow and internal diameter between the two groups at 24 hours after operation(P>0.05). At 3 months after operation, the cephalic vein blood flow and internal diameter of the two groups were higher than those of the control group at 24 hours after operation(P<0.05), and the observation group was higher than the control group(P<0.05). The pain rate(66.67%) at 3 days after operation in the observation group was lower than that in the control group(93.33%, P<0.05). The physiological maturity rate(93.33%) and clinical maturity rate(86.67%) of internal fistula in the observation group were higher than those in the control group(83.33%, 80.00%), but there was no statistically significant difference(P>0.05). The total incidence of complications in the observation group(10.00%) was lower than that in the control group(36.67%, P<0.05). Conclusion Early mild moxibustion can improve the function of internal fistula after autologous AVF in hemodialysis patients, improve the maturity rate of internal fistula, relieve the pain of operation site, and reduce the clinical complications.
