摘　　要：目的对比分析新型固定环联合羊膜移植与传统羊膜移植遮盖方式治疗角膜溃疡的临床效果,以期为临床找到一种更为安全简单有效的方式。方法选取2016年2月至2018年10月于我院行羊膜移植术的64例角膜溃疡患者作为研究对象,根据手术方式不同,将其分为观察组与对照组,每组32例。观察组患者采用新型固定环联合羊膜移植治疗,对照组采用传统羊膜移植遮盖方式治疗。比较2组患者手术时间,术中及术后1d、3d、5d疼痛程度评分及羊膜存活时间。结果观察组患者手术时间少于对照组,差异具有统计学意义(P<0.05);对照组患者术中及术后1d、3d疼痛程度评分均高于观察组,差异具有统计学意义(P<0.05);术后5d疼痛程度评分2组患者对比差异无统计学意义(P>0.05);观察组患者平均羊膜存活时间为(10.82±2.26)d,对照组为(5.72±1.14)d,差异有统计学意义(P<0.05)。结论新型固定环联合羊膜移植术能提高角膜溃疡患者舒适度,减轻术中及术后疼痛,显著缩短手术时间,有效提高羊膜的存活时间,优于传统的羊膜移植方式。Objective To compare and analyze the clinical effect of new fixed ring combined with amniotic membrane transplantation and traditional amniotic membrane transplantation in the treatment of corneal ulcer,in order to find a more safe,simple and effective way for clinical treatment.Methods A total of 64 patients with corneal ulcer who underwent amniotic membrane transplantation in our hospital from February 2016 to October 2018 were selected as the study subjects.According to the different surgical methods,they were divided into observation group and control group,32 cases in each group.The patients in the observation group were treated with new fixed ring combined with amniotic membrane transplantation,while the patients in the control group were treated with traditional amniotic membrane transplantation.The length of the operation,the pain degree score during surgery and on 1st,3rd and 5th day after surgery,and the duration of amniotic membrane survival were compared between the two groups. Results The time of operation of observation group was shorter than that of the control group,the difference was statistically significant( P <0.05).The pain score during surgery and on 1st ,3rd day after surgery were higher in control group than those in the observation group,the difference was statistically significant( P <0.05).But there was no statistically significant difference between the two groups in the 5-day postoperative pain score( P >0.05).The average amniotic membrane retention time was (10.82±2.26)days in the observation group and (5.72±1.14)days in the control group,the difference was statistically significant between the two groups( P <0.05). Conclusion The new fixed ring combined with amniotic membrane transplantation can improve the comfort of patients with corneal ulcer,avoid intraoperative and postoperative pain,significantly shorten the operation time,and effectively improve the survival time of amniotic membrane,which is superior to traditional amniotic membrane transplantation methods.
