AECOPD患者平喘乳膏穴位贴敷配合雾化治疗应用研究

摘　　要：目的探讨平喘乳膏穴位贴敷配合雾化治疗对慢性阻塞性肺疾病急性发作期(AECOPD)患床应用效果。方法选取2017年01月~2019年05月南京中医药大学扬州附属医院重症医学科收治的AECOPD患者42例作为研究对象,其中行无创正压通气辅助呼吸治疗的20例患者为对照组,另外行气管切开间歇指令通气辅助呼吸治疗的22例为观察组。两组均给予布地奈德+特布他林雾化雾化吸入治疗,观察组配合平喘乳膏穴位贴敷。干预2周,对比两组临床咳嗽症状改善情况及炎性指标变化。结果干预前,两组咳嗽症状评分及炎性指标血清白介素(IL-8)、血清超敏C-反应蛋白(CRP)水平比较,差异无统计学意义(P>0.05);干预后,两组患者咳嗽症状评分及IL-8、CRP对比干预前均有明显改善,但观察组改善程度均比对照组明显,观察组咳嗽症状改善有效率明显高于对照组,差异均有统计学意义(P<0.05)。结论对AECOPD患者实施平喘乳膏穴位贴敷配合布地奈德雾化治疗,患者炎性因子水平明显降低,临床咳嗽症状明显缓解,效果显著。Objective To explore the effect of acupoint application of acupuncture cream combined with atomization therapy on the bed of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 42 patients with AECOPD admitted to our hospital from January 2017 to April 2019 were enrolled.Among them, 20 patients underwent noninvasive positive pressure ventilation assisted respiratory therapy as the control group,and 22 patients underwent tracheotomy intermittent mandatory ventilation assisted respiratory therapy.For the observation group.Both groups were given budesonide + terbutaline atomized aerosol inhalation treatment,and the observation group was combined with Pingchuan cream acupoint application.After 2 weeks of intervention,the improvement of clinical cough symptoms and changes of inflammatory indexes were compared between the two groups.Results Before the intervention,there was no significant difference in serum cauline(IL-8) and serum high-sensitivity C-reactive protein(CRP) between the two groups(P>0.05).After the intervention,the cough symptom scores of the two groups were compared.Compared with IL-8 and CRP,the improvement of the observation group was significantly higher than that of the control group.The improvement of cough symptoms in the observation group was significantly higher than that of the control group;the difference was statistically significant(P>0.05).Conclusion Acupuncture cream applied to AECOPD patients combined with budesonide atomization treatment,the level of inflammatory factors was significantly reduced,clinical cough symptoms were significantly relieved,and the effect was significant.
