不同表观扩散系数与乳腺癌淋巴管侵犯的相关性研究

摘　　要：目的:探讨不同表观扩散系数(ADC)与乳腺癌淋巴管侵犯(LVI)的相关性。方法:对本院87例乳腺癌患者的磁共振及临床病理资料进行回顾性分析。所有患者均行常规磁共振成像以及扩散加权成像(DWI);分别测量并获取乳腺癌病灶最小ADC值(Tumor-ADC min )、瘤周腺体最大ADC值(Peritumor-ADC max ),并计算两者比值ADC ratio 。以术后病理结果为金标准,分别比较LVI阳性组以及LVI阴性组Tumor-ADC min 、Peritumor-ADC max 及ADC ratio 的差异。结果: 87例乳腺癌患者,其中LVI阳性51例,阴性36例。LVI阳性组Tumor-ADC min 值低于LVI阴性组( t =3.311, P =0.001),分别为(0.91± 0.26)×10^-3 mm^2/s、(1.13±0.37)×10^-3 mm^2/s;LVI阳性组Peritumor-ADC max 值高于LVI阴性组( t =-2.054, P =0.043),分别为(1.57±0.31)×10^-3 mm^2/s、(1.44±0.29)×10^-3 mm^2/s;LVI阳性组ADC ratio 值亦高于LVI阴性组( t =-4.532, P <0.001),分别为1.81±0.43、1.38±0.42。结论:不同ADC值对乳腺癌LVI具有较高的预测价值,可为评估乳腺癌LVI提供参考依据。Objective: To explore the value of different apparent diffusion coefficients (ADC) in predicting lymphovascular invasion (LVI) of the breast cancer. Methods: The clinical and pathological data of 87 patients with breast cancer in our hospital were retrospectively analyzed.The minimum ADC value of tumors(Tumor-ADC min ) and the maximum ADC value of peritumor were measured,and the ratio of them (Peritum-ADC max /Tumor-ADC min ) was calculated,which were defined as the ADC ratio .The differences of Tumor-ADC min ,Peritumor-ADC max and ADC ratio between LVI positive and negative group were compared. Results: Among 87 cases of the breast cancer,51 cases were LVI positive and 36 cases were LVI negative.Tumor-ADC min value in LVI positive group was lower than that in LVI negative group [(0.91±0.26)×10^-3 mm^2/s vs (1.13±0.37)×10^-3 mm^2/s, t =3.311, P = 0.001]. Peritumor-ADC max value in LVI positive group was higher than that in LVI negative group [(1.57±0.31)×10^-3 mm^2/s vs (1.44±0.29)×10^-3 mm^2/s, t =-2.054, P =0.043].ADC ratio value in LVI positive group was also higher than that in LVI negative group [(1.81±0.43) vs (1.38±0.42), t =-4.532, P <0.001]. Conclusion: Different ADC values have high predictive value for LVI of the breast cancer,which may provide a reference for quantitative evaluation of LVI of the breast cancer.
