摘　　要：目的总结胎儿镜像右位心的产前超声特征。方法回顾性分析37例经产前超声诊断为镜像右位心胎儿的临床资料,总结产前超声图像特征,分析其诊断方法及漏诊原因。结果产前超声心脏四腔心切面显示心脏大部分位于右侧胸腔,心底与心尖连线指向胎儿右侧,心房反位,心室左袢。37例镜像右位心胎儿均合并腹腔内脏反位。11例(29.7%)无结构异常,21例(56.8%)合并心脏结构异常,5例(13.5%)合并心外结构异常,2例(5.4%)同时合并心内外结构异常。结论系统规范的产前超声检查可以发现并诊断胎儿镜像右位心。胎儿镜像右位心合并其他畸形的发生率较高。Objective To summarize the prenatal ultrasonic features of fetal mirror image dextrocardia.Methods The clinical data of 37 fetuses diagnosed as mirror image dextrocardia by prenatal ultrasonography were retrospectively analyzed,their prenatal ultrasonic image features were summarized,and their diagnostic method and reasons for missed diagnosis were analyzed.Results Prenatal ultrasonography revealed that the heart mostly located in the right thoracic cavity,and the connection between basis and apex pointed to the right side of the fetuses,with atrial situs inversus and ventricular levo-loop,in apical four chamber view.Complication with situs inversus viscerum of abdominal cavity was found in all of the 37 fetuses with mirror image dextrocardia.No structural anomalies were observed in 11(29.7%)cases,complication with cardiac structural anomalies in 21(56.8%)cases,complication with extracardiac structural anomalies in 5(13.5%)cases,and complication with cardiac and extracardiac structural anomalies in 2(5.4%)cases.Conclusion Systematic and regulatory prenatal ultrasonography can identify and diagnose fetal mirror image dextrocardia.The incidence of complication with other anomalies is relatively higher in fetal mirror image dextrocardia.
