少儿原发性门静脉海绵样变的超声诊断价值

摘  要:目的探讨超声在少儿原发性门静脉海绵样变中的诊断价值,以提高对本病的认识。方法2010年1月至2017年10月在四川大学华西医院诊断为原发性门静脉海绵样变,病历资料完整的患儿35例,根据门静脉变性累及的范围进行分型,分析各型的超声声像图特点。结果根据门静脉海绵样变累及的范围分为3型。Ⅰ型15例,为门静脉主干病变,Ⅱ型14例,为门静脉主干及肝内门静脉病变型,Ⅲ型6例,为肝内门静脉病变型。与正常少儿比较门静脉管径之间差异有统计学意义(P<0.05)。Ⅲ型病变更易合并其它先天性畸形。结论超声能较好地显示少儿原发性门静脉海绵样变的直接和间接征象,以及血流动力学的改变,是诊断和随访的首选方法,具有较高的应用价值。Objective To investigate the diagnostic value of ultrasound in children with primary cavernous transformation of the portal vein in order to improve the understanding of this disease.Methods From January 2010 to October 2017,35 children patients with primary cavernous transformation of the portal vein and complete medical records in West China School of Medicine,Sichuan University were enrolled.The characteristics of various ultrasound images were analyzed according to the range of portal vein degeneration.Results According to the range of cavernous transformation of the portal vein,it was divided into 3 types.There were 15 cases of type Ⅰ,which had main portal vein lesions,14 cases of type Ⅱ,which had main portal vein and intrahepatic portal vein lesions,6 cases of type Ⅲ,which had intrahepatic portal vein lesions.There was a statistically significant difference in the diameter of the portal vein compared with normal children( P<0. 05).Type Ⅲ disease is easily combined with other congenital malformations.Conclusion Ultrasound can better display the direct and indirect signs of primary cavernous transformation of the portal vein in children,as well as hemodynamic changes,which is the first choice for diagnosis and follow-up,and has high application value.
