三维超声冠状面对宫腔粘连的诊断价值

摘　　要：目的探究经阴道三维超声冠状面对宫腔粘连的临床应用价值.方法选择2017年12月至2018年3月在上海中医药大学附属岳阳中西医结合医院就诊的30例疑似宫腔粘连患者,均给予经阴道二维超声及三维超声冠状面检查,结果与宫腔镜检查相比较,判断二维及三维超声对宫腔粘连的诊断价值.结果二维和三维超声诊断准确率分别为73.3%(22/30)、93.3%(28/30),阳性预测值分别为84.0%(21/25)、96.0%(24/25);二维和三维超声诊断宫腔粘连受试者工作特征曲线(receiver operating characteristic curve,ROC)曲线下面积分别为0.520、0.880,渐进P值分别为0.889、0.008.结论三维超声冠状面能直观清晰显示宫腔的特征,相比二维超声更具有临床诊断价值.Objective To explore the diagnostic value of the transvaginal three-dimensional ultrasound coronal plane in patients with intrauterine adhesion. Methods Thirty cases of suspected intrauterine adhesion were chosen and all underwent transvaginal two-dimensional and three-dimensional ultrasonography coronal plane examination. The diagnostic value of the two-dimensional and three-dimensional ultrasonography in intrauterine adhesions was evaluated as compared with that of hysteroscopy. Results The diagnostic accuracy of the two-dimensional and three-dimensional ultrasonography were respectively 73.3%(22/30)and 93.3%(28/30), and the positive predictive values were respectively 84.0%(21/25) and 96.0%(24/25). The area under the ROC curves of the two-dimensional and three-dimensional ultrasonography were respectively 0.520 and 0.880, and the progressive P values were 0.889 and 0.008 respectively. As compared with the two-dimensional ultrasonography, the three-dimensional ultrasonography had a higher diagnostic value for the diagnosis of intrauterine adhesion. Conclusion The feature of the three-dimensional ultrasonography coronal plane was more direct and clear, and was of better diagnostic value, as compared with the two-dimensional ultrasonography.
