3D超声断层显像评价瓜蒌牛蒡汤联合加味双柏膏治疗乳痈早期临床研究

摘　　要：目的:应用3D超声断层显像技术评价瓜蒌牛蒡汤联合加味双柏膏治疗乳痈早期患者的疗效。方法:将50例乳痈早期气滞热壅证患者随机分为对照组和观察组各25例。对照组给予瓜蒌牛蒡汤治疗,观察组给予瓜蒌牛蒡汤内服联合加味双柏膏外敷治疗,1周为1个疗程。治疗1个疗程后,统计2组的临床疗效,治疗前后评定中医证候积分及应用3D超声断层显像技术测定病灶灰阶值中位数(GSM)。结果:治疗后,观察组总有效率92.0%,对照组总有效率64.0%,2组比较,差异有统计学意义(P <0.05)。2组中医证候积分均较治疗前下降(P <0.01),观察组中医证候积分低于对照组(P <0.01)。2组病灶GSM值均较治疗前升高(P <0.01,P <0.05),观察组病灶GSM值高于对照组(P <0.05)。结论:应用瓜蒌牛蒡汤内服联合加味双柏膏外敷治疗乳痈早期疗效明确、安全有效,联合用药能提高临床治疗有效率,并加速乳痈病灶灰阶值的恢复。Objective: To evaluate the curative effect of the application of Gualou Niubang tang combined with modified Shuangbai ointment for early mammary abscess by using 3 D ultrasound tomographic imaging. Methods:Divided 50 cases of patients with early mammary abscess with qi stagnation and heat congestion syndrome randomly into the control group and the observation group. The control group was treated with Gualou Niubang tang,and the observation group was treated with Gualou Niubang tang orally combined with the external application of modified Shuangbai ointment for 1 week as a course.Recorded the clinical effect of the two groups after 1 course of treatment, evaluated Chinese medicine syndrome scores before and after treatment, and detected the gray-scale median of lesions(GSM) by using 3 D ultrasound tomographic imaging. Results: After treatment, the total effective rate was 92.0% in the observation group and 64.0% in the control group,compared the two groups,difference being significant(P < 0.05). The Chinese medicine syndrome scores in the two groups were lower than those before treatment(P < 0.01);the Chinese medicine syndrome scores in the observation group were lower than those in the control group(P < 0.01). The value of lesions GSM in the two groups were higher than those before treatment(P < 0.01,P < 0.05);the value of lesions GSM in the observation group was higher than that in the control group(P < 0.05). Conclusion:The application of Gualou Niubang tang orally combined with the external application of modified Shuangbai ointment for early mammary abscess has obvious curative effect,which is effective and safe. Concomitant drugs can improve the efficiency of clinical treatment and accelerate the recovery of gray scale of lesions.
